Your space in class is ONLY reserved once this enrollment form and full payment are received.

LIABILITY WAIVER / RELEASE: Owner agrees to release Caliber Canines from all liability for any loss, damage or injury to persons, animals or property arising from or related to Owner's pet. Owner agrees that Caliber Canines shall not be liable for loss or damage to animal for any reason. Payment in full will be made prior to each lesson via cash, check or money order unless other arrangements have been made with Caliber Canines. Checks are cashed upon receipt. Prior to the start date of class refunds will be granted minus a nonrefundable $15 deposit. Caliber Canines is fully insured by the CNA Insurance Company.

Before you come to class . . .

What to bring

1. A copy of your dog's rabies certificate for High Goal Farm to keep
2. Water for both you and your dog
3. Wear comfortable shoes as you will be standing for most of the class
4. LOTS and LOTS of your dogs very favorite treats. Something soft and stinky is best. If your dog is motivated by toys those are great too. Please try not to feed your dog within 4 hours of class, a hungry pooch is much more eager to earn rewards!
5. Have your dog on a regular collar or harness. Head collars such as a Halti or Gentle Leader are great, as are harnesses such as the Premier Easy-Walk. Absolutely no metal correction collars (such as a prong or choke chain) are allowed, this is a positive training class and zero exceptions will be made. Talk to Tara to have your dog fitted for a Gentle Leader or Harness.
6. A 4-6ft leash, no flexi's.
7. Poop bags (we supply the trash bins!)

8. A Clicker if you use one (NOTE: Clickers are included for the CLICKS & TRICKS, Canine Manners and Puppy FUNdamentals classes.)
