Instructions:
Choose the event being recorded and provide the date of the transaction. If the event is a theft and a police report has been filed, list the police jurisdiction and report ID at the bottom of this form to aid in recovery of the bird. Record the bird’s ID number and type of ID. Record the gender (M, F, ?) and type (DNA, endoscopy). If this event is a breeding loan, use “current Owner” to record who the bird is loaned to and “Previous Owner” to designate who is loaning the bird.
Species – P. sordidus (Coral-Billed), P. menstruus (Blue-headed), P. chalcopterus (Bronze-winged), P. fuscus (Dusky parrot), P.maximiliani (Maximilian’s Pionus), P. tumultuous (Plum-crowned), P. senilis (White-crowned ), P. seniloides (Massena’s parrot, White-headed), or hybrid (if hybrid, please add parent species information on back of form). Sub-species – Pionus sub-species designations are appropriate for Coral-billed (mindoensis is no longer considered a sub-species), Blue-headed, and Maximilians. The Bronze-winged sub-species designations no longer apply. Do not designate a sub-species unless you know that the bird has the same morphological characteristics as its parents.
The F numbers refer to generations. For example, if a bird is a wild-caught, it is an F0. If its parents were wild caughts, it’s an F1. If its parents were F1’s, then the bird is an F2. If a bird is the product of a mixed generation breed, then its number is 1 plus the largest parent designation – for example, the chick of an F1-F2 cross would be an F3. DO NOT provide this information if you are in doubt as to the birds’ lineage.
