Guide for Entering Primary Referral Sources in the ITS Database
In the ITS Database, entry of primary referral sources is managed through the File Maintenance option on the first screen seen after log-in. These referral sources are defined in federal regulation, and are useful for local analysis of the community sources for child find activities.
Primary referral sources and categories (per federal regulations):
Categories are underlined, source type examples listed below
Doctor/Physician
Including local individual or group primary practitioners, and specialists (e.g., Audiologists, Cardiologists, Ophthalmologists, Psychologists)
Hospital
Including prenatal and postnatal care facilities
Neo-natal Intensive Care Unit (NICU)
Parents/ Family/ Friends
Including extended family (aunts, uncles, grandparents, etc.), foster parents, and guardians
Child care and early learning programs
Including public or private child care (day care) providers (individuals or centers),
Head Start,
Early Head Start,
Families Together, Inc,
Project Eagle (KUMC), and
Other Part C local programs
Public Health Facilities
Including Local Health Departments,
Public clinics,
Community mental health centers (CMHCs),
Federally Qualified Health Centers (FQHCs) / Rural Health Clinics (RHCs), and
Indian Health Centers (IHS or IHCs)
Local Education Agencies
Including school districts,
Cooperatives,
Inter-locals,
Parents as Teachers, and

It is recommended that the referral sources entered into the database be double checked when reviewing the semi-annual reports in January and July of each year (Question 3) to see if some sources have been inappropriately placed in the “other” category.
To categorize correctly, review your list.
Under Referral Source: Include your local referral source, name, agency, etc.
Under Category: Include the Federal Category as described above.
If you are no longer using a particular referral source, you may click the inactive button, and that source will not appear in your dropdown. See the screen shot below:
