To Author/Editor: This document is an important organizational tool for documenting the third-party material and all photos included in your manuscript. Completeness and accuracy on this log will help streamline development and production, as well as simplify tasks for any future editions.

In accordance with your author/editor agreement, you have warranted that you will identify and obtain all required permissions and/or releases needed for any third-party material contained within the manuscript. In addition, for all photographs you have personally taken, you must also obtain a visual likeness release from any identifiable person(s) in the photo. Please submit a permissions log listing full source information for all third-party material along with your final manuscript. All permissions must be requested before transmittal so copyediting can begin on schedule. All permissions must be obtained with a final, complete, signed log sent to your Editorial Team in order for typesetting to commence (or the paging and/or publication of your project may be delayed).

Please be sure to include a full citation to the original source within your manuscript for ALL third-party items, including those in the public domain or deemed fair use for which permission is not required.

Chapter – Chapter in which material appears.

Item Name and Description – Description of item being used to aid locating it in the manuscript.

Full Source Information – Complete source/reference for the original publication of the material used (this information will be used to create the credit line if a credit is not provided on the perm agreement).

Copyright Holder – The publisher, creator or other party who holds copyright to the original material, including contact info.

Date Permission Requested – Date permission was requested.

Date Permission Received – Date stated on permission agreement when permission was granted.

Print Rights and any Restrictions - Complete this information using the terms specified in the permission agreement. Unless your Editorial team instructs you otherwise, all permissions received must include the following rights: Worldwide, English language rights, in all formats and media, and for all editions. Any restrictions indicated on the copyright holder’s agreement must be noted on the log, and your Editorial team should be notified immediately. If there are no print restrictions, please include the words “no restrictions.”

E-Rights and any Restrictions - Permission must include electronic use, unless previously arranged with your Editorial team. Indicate the terms of electronic use specified in the permissions agreement, and any restrictions, i.e. limited quantities, etc. indicated on copyright holder’s agreement should be noted and your Editor should be notified immediately. If there are no restrictions, write the words “no restrictions.” Please note that if the rightsholder requires that you specify a type of electronic rights in your permission request, you should specify: “e-Book and access restricted website.”

Fee/Comp. Copy – Indicate the fee and/or comp. copy due to Rightsholder as stated on the permissions agreement. The Editorial team will order all comp. copies upon publication.

Comments/Status of Request – This column is for comments regarding the status of your request or questions about an item for your Editorial team to review.

Credit Line – If the permissions agreement specifies a required credit line, then please write the EXACT credit line in this space. If no specific credit line is required under the permissions agreement, then please note “Production to create credit line,” and your Production Editor will create a credit line using the information you provided in the “Full Source Information” column. If the rights holder has indicated that they do not wish to be credited, write “Do not credit.” Do not leave this column blank.
