This is a sign out sheet to verify that ___________________________________ is asking to borrow the Game Base 7 Laptop for use at a gaming related event. The said borrower is now the responsible party for the club laptop. If anything happens to the club laptop, the borrower will replace the club laptop at the cost of $250.00 or pay the repair bill up to the cost of $250.00. Below will be the dates asked for and the signature of the Systems Chairman or President of GB7 and the said borrower. There will also be a date of return which will be signed back to the club and dated. That said date will be the day that said borrower will no longer be responsible for the club laptop at that time.
