Managers, Coaches & Helpers Kit:

Qualified (FA Level 1 & above) Team Managers & Coaches are entitled to Club drill tops, Club wet topsand are also entitled to the Coaching uniform.

Qualified (FA Child Protection) Coaching Parent Helpers are entitled to Club drill tops & Club wet tops.

All kit is issued free of charge to Managers, Coaches and Helpers – but it must be returned if you leave the Club.

Please pass one copy of this completed & authorised form to Gary and retain the other for your information. Orders over £150.00 have to be authorised by 3 Committee members, any very large orders may require authorisation at the monthly Management Committee meeting.

All kit orders will be confirmed by email fromGary Johns. (), an expected delivery date will be advised once the order has been placed with our suppliers.

Garywill email the Team Manager to arrange for collection as soon as an order has been received.
