I/We, ______the undersigned, are the parents/legal guardians of______.

I/We desire that our daughter be allowed to participate in the Nor’ Easter Softball Club and its athletic and other activities. We recognize that athletic competition inherently entails some risk of injury and freely acknowledge our intent that our daughter participate in the Association/s activities with full knowledge of that risk.

In consideration of our daughter being permitted to participate in the activities of the Association, including but not limited to tryouts, practices, games, tournaments and other activities of us and/or our daughter being permitted to compete, officiate, observe, or otherwise participate in such activities, we, for ourselves, our daughter, and our heirs and assigns:

1. Hereby release, waive, discharge, and covenant not to sue the Nor’ Easter Softball Club, its principals, officers, directors, coaches, umpires, staff volunteer workers, or other representatives from all liability to us and/or our daughter for any and all loss or damage, and any claim or demand therefore on account of injury to the person or property of ourselves or our daughter, however caused, whether caused by the negligence of the Nor’Easter Softball Club or its representatives as listed above, or otherwise, while I/We or our daughter is in or upon the premises used by the Nor’Easter Softball Club, and/or competing or officiating in, observing, working for, or for any purpose participating in any activities of, the Nor’ Easter Softball Club, including transportation to and from such activities, and also including any other activities including the maintenance or improvement of the premises occupied or operated by the Nor’Easter Softball Club.

2. I/We hereby assume full responsibility for and risk of bodily injury, death or property damage due to the negligence of the Nor’Easter Softball Club or any other representatives as listed above, or otherwise, while in or upon the premises used or occupied by the Association and/or while competing, officiating observing or working for the Association or for any purpose participating in any of the activities of the Association.

3. I/We hereby agree to indemnify and save harmless the Nor’ Easter Softball Club, its principals, officers, directors, coaches, umpires, staff; volunteer workers, or other representatives from any law suits, claims or demands of any kind brought forward by our daughter, ourselves, or heirs and assigns, or any other party against the aforementioned Association and individuals listed above.

I/WE THE UNDERSIGNED HAVE READ AND VOLUNTARITY SIGNED THIS RELEASE,

and further agree that no oral representations, statements or inducements, apart from the forgoing written agreement, have been made to us.

I/We hereby authorize the Nor’Easter Softball Club coaches, assistants, and volunteers to act for me, according to their best judgment in an emergency requiring medical attention, and hereby waive, release, and indemnify Nor’Easter Softball Club, its coaches, assistants, and volunteers from any liability and cost for any injuries incurred while in this program. I/We assume responsibility for payment for any such attention.

This material may also appear on the Nor’Easter Softball Club Internet Web Page. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Player Code of Conduct Outline

Players and parents please understand that travel softball is a highly competitive sport and there are no guarantees as to the amount of playing time or position that you will receive. Be aware that the club will have a limited membership, and as such that each player’s participation is crucial in maintaining the level of play that we are striving to achieve.

Understand that it is your responsibility to attend mandatory team functions, abide by the coaches directives while at team functions and to notify coaches if not able to attend. Finally, understand that the goals of playing for the Nor’Easter Softball Club are to have fun, develop softball skills and help foster sportsmanship and a positive team spirit.

By accepting a position with the Nor’Easter Softball Club you would agree to:

  • Respect the game, my teammates, my coaches, the opponent, the facilities, the umpires, my parents and spectators.
  • Demonstrate sportsmanship at all times.
  • Make every effort to show up on time for practices, games and team events.
  • Work hard for the benefit of yourself and for the benefit of the team by giving best effort in practices and games – hustle all of the time.
  • Lead by example for my teammates.
  • Use positive language to cheer and support my teammates.
  • Follow the direction of the coaching staff and support a coach’s decision.
  • Settle disagreements with coaches and other players in a confidential/ private manner after games and practices.
  • Refrain from throwing bats, gloves, helmets or other equipment for any reason.
  • Understand that while winning is desirable, it is not the only yardstick used to measure our successes.
  • Never use profane language at any practice, game or team event.
  • Never fault a teammate when a mistake occurs.

These statements reflect the philosophy of our club and ASA Softball. Violations of above policies can be subject to discipline as outlined in our bylaws.
