DISCLAIMER OF LIABILITY & STUDENT AGREEMENT

WHEREAS the undersigned voluntarily desires to participate in this activity; the UNDERSIGNED acknowledges that Texas Woman's University has limited sponsorship of this trip; that TWU's only interest in this activity is to make the trip available. TWU expressly disclaims any liability for personal injury or property damage or loss arising out of the act, omission, or negligence of the undersigned;

WHEREAS the undersigned is duly aware of the risks and hazards which may arise through participation in such activity and that participation in such activity may result in loss of property and/or life and/or limb of the undersigned:

WHEREAS the undersigned is duly aware of the risks of staying in a facility not affiliated with the university and that risks may include fire hazards;

THEREFORE, it is agreed as follows:

THAT in consideration of the undersigned's participation in such activity, the undersigned hereby voluntarily assumes all risks of accident or personal injury, including injuries unknown and unanticipated, or death, to his/her person or property and hereby releases and forever discharges the Texas Woman's University, its Board of Regents, officers, agents, employees from any and all liability arising from or connected with the undersigned's participation in such activity.

THAT the undersigned assumes full responsibility for her/his actions and those actions of her/his guest(s) and in the event of damage or disciplinary problems, will be responsible for compensation of damages to TWU and subject to disciplinary action by the TWU Vice President for Student Life or his/her designee.

THAT if the undersigned drives him/herself to and/or from said event, it is his/her personal choice. By making this decision, she/he assumes full responsibility and agrees to participate fully in the said event.

THAT the undersigned will educate him/herself about fire exits if an event is held at an off-campus facility.

THE UNDERSIGNED, by signing this release, hereby certifies that he/she has read and fully understands the conditions herein provided.

Refund Policy: Full refunds will be given if: the trip is canceled due to lack of participation or weather, or if 72 hour notice is given to the Center for Student Development prior to the trip's departure. In those cases of trips requiring pre-trip meetings, refunds will be given if notice is given 72 hours prior to meeting time. All trips will depart from the Student Union unless otherwise stated. Participants are to meet in front of the Student Union at the time designated. Any questions concerning trips' policies or procedures are to be directed to the Director of the Center for Student Development.
