The Psychotherapy Specialty Doctor post is an established post. It is based in the Lansdowne Psychotherapy Service. Duties will be allocated by the Consultant Psychiatrists in Psychotherapy in the light of the post holder’s experience and the needs of the psychotherapy service. Clinical accountability will be to the Consultant Psychiatrists in Psychotherapy. Duties will involve clinical assessment of new referrals to the service and treatment of patients under appropriate supervision.

Excellent communication skills are essential.The post is linked to active teaching programmes and continuing professional development, audit and research are encouraged and supported.You should have full medical registration and experience of working in Psychiatry.

The General Adult posts are offered on a part time basis (5 sessions).

Sessional work for the post is between the hours of 9am and 5pm Tuesday - Thursday.

All applicants must possess full GMC registration, a licence to practice and have at least 4 Years full time Post Graduate Training, two of which must have been in relevant acute specialties, or you will demonstrate and evidence equivalent experience and competencies.

Specialty Doctor in Psychodynamic Psychotherapy: Lansdowne Psychotherapy Service

1. Population served

The Lansdowne Clinic provides a Psychodynamic Psychotherapy Service to the West Sector of Glasgow (population 190,000). A recent audit of referrals showed 26% of patients come from areas categorised as highly deprived (Depcat score 6 or 7) with 41.9% of patients coming from areas of middle deprivation (Depcat score3, 4 and 5).

2. Psychiatry and Psychotherapy Services

There are five psychotherapy services in NHS Greater Glasgow and Clyde Health Board Area, based in the West, North, East and South of Glasgow and one psychotherapy service based in Dykebar hospital Paisley (Clyde). There is also a Consultant Psychiatrist in Psychotherapy leading the Glasgow-wide “Homelessness Personality Disorder Service” and a Consultant Psychotherapist in West Dumbarton. The Consultant Psychotherapists from these services form the Division of Psychotherapy, a subcommittee of the Divisional Psychiatric Advisory Committee.

The Lansdowne Clinic psychotherapy service forms part of the overall psychiatric provision for West Glasgow and provides the psychodynamic psychotherapy resource for the area. Community psychiatric services are delivered by integrated (health and social care) teams based in community resource centres, and there are a number of specialist teams, for example, early intervention in psychosis (ESTEEM teams), liaison psychiatric services, community addiction services, mental health services to asylum seekers, services to patients with dual diagnoses, psychiatric services to the homeless population, crisis intervention services, primary care mental health teams etc.

The psychotherapy service is located within the Centre for Health and Care, Partick, along with the Riverside CMHT, Sandy Road Primary Care Team and other community services. This is a newly built facility in Sandy Road a short distance from Gartnavel Royal Hospital (GRH) where in-patient beds for the sector are located.

The clinic receives referrals from primary and secondary care services.New patients are assessed by senior (specialist) staff and by trainee psychiatrists under close Consultant supervision. The patient group is primarily those with mood disorders, post traumatic disorders, neurotic disorders and personality disorder. Patients are usually significantly more disturbed than those treated in primary care settings and often have received previous treatment, including in patient admissions. The treatment model is that of psychodynamic psychotherapy.

Within the department, in addition to the general psychotherapy clinic is a clinic for patients who have been sexually abused.

Careful assessment of patients’ difficulties is the cornerstone of treatment planning; thereafter treatments offered include individual psychodynamic psychotherapy, brief dynamic therapy, supportive psychotherapy, psychodynamic group or couple therapy and mentalisation based therapy.

The department has a key role in providing training and supervision in psychotherapy to psychiatric trainees. There is currently an Higher Psychiatry Trainee on placement within the service.

The department has a wider role within the psychiatric service, offering consultation, support advice and training where appropriate to staff working within the inpatient and community services.

3. Annual Workload

The psychotherapy service receives an average of 360 referrals per year. A steady proportion of these are to the sexual assault clinic, which amounted to 50 new referrals in the last 12 months.

The sexual assault clinic was established within the Lansdowne Clinic psychotherapy service in the 1980s in response to growing awareness of the scale of sexual abuse and of its short and long term sequelae. The clinic offers expertise in assessment and treatment of patients who have been sexually abused or assaulted. It is a valuable resource for the psychiatric services, voluntary sector and for general practice. Many of the patients referred have substantial psychiatric histories therefore skilled clinical assessment is crucial to the provision of appropriate therapeutic help.

The post will be based within the Lansdowne Psychotherapy Service. The successful applicant should have some experience of psychiatric assessment and an interest in psychodynamic psychotherapy . Training and supervision will be provided which could include Mentalisation Based Therapy.

4. Duties of the Post

Clinical

The Psychotherapy Specialty Doctor post is an established post and recently vacated. It is based in the Lansdowne Psychotherapy Service. Duties will be allocated by the Consultant Psychiatrists in Psychotherapy in the light of the post holder’s experience and the needs of the psychotherapy service. Clinical accountability will be to the Consultant Psychiatrists in Psychotherapy. Duties will involve clinical assessment of new referrals to the service and treatment of patients under appropriate supervision.

Teaching

GartnavelRoyalHospital is a teaching hospital and, as such, it aims to provide high quality teaching for all health care personnel. Medical students from GlasgowUniversity are attached to the consultant psychiatrists on a 5 week clinical clerkship basis. The students receive teaching in both the hospital and community settings to enable as rounded an experience in psychiatry as possible. The staff grade psychiatrist will be expected to participate, under supervision, in informal and formal teaching of medical students, as well as teaching students of nursing, social work, and occupational and other therapists. The specialty doctor would be encouraged to participate in audit and research and ongoing projects within the service under supervision fulfilling requirements for annual appraisal.

6. Continuing Education

The postholder will be encouraged to utilise his/her full entitlement to study leave. Attendance at fortnightly case conference/journal clubs would be expected or individuals could negotiate attendance at the external MRCPsych course, if they were planning to sit exams, or this was required for their Continuing Professional Development. The facilities of the main library at GartnavelRoyalHospital will be available. There is also an online library, which includes a literature search facility, to which all staff may have access. Advice on continuing postgraduate education and careers counselling is available from both the college tutors and other senior staff.

7. Management and Administration

The post holder will be encouraged to help formulate policies with regard to the development of the Psychotherapy Service and its role within the mental health services.

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

SUPERANNUATION / New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme. Should you choose to "opt out" arrangements can be made to do this via:
REMOVAL EXPENSES / Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.
