Please make sure that you:

·  Pay the tariffs and clearly state the PUC on the proof of payment

·  Keep a copy of the whole registration document and records of the Courier Service used to send the documentation

·  Study the registration forms carefully and complete it as requested

NB: The tariff payment is R 400. 00 once off per PUC (No tariff payments for preliminary USA table grapes, the payments

will be done at the later stage after the agreement with USDA)

1. Declaration of intent:

I/We the undersigned as producer understand and shall comply with all the stipulations of the applicable export programme(s) (protocols) with specific reference to the following:

1.  Only fruit produced in registered vineyards may be sourced for the applicable export programme(s).

2.  Consignment shall be free of any of the listed quarantine organisms that pose a phytosanitary risk to the importing country.

3.  Only DOA and PPECB approved packing houses and cold chambers/containers must be utilised for handling, storing and cold sterilisation of the fruit.

4.  Only fruit that are from the approved production unit (s) (PUC) and clearly marked for a specific export programme shall be allowed for export.

5.  Application for registration of production unit shall not be considered if

a)  The registration document is illegible (information to be typed or completed in clear block letters),

b)  Information is insufficient or incorrect, or

c)  Documentation is received after the closing date,

d)  When documentation is mailed or faxed, and

e)  When there is no proof of tariff payments.

6.  Pages must be signed at the indicated places, and initialed at the bottom of each page of this document.

7.  Appendix A must be completed by only producers intending to export table grape to Israel (Page 5-8)

8.  The Directorates Plant Health and or APIS shall not be held responsible for any financial or other losses resulting from non-compliance by the producer or the exporter agents providing incorrect, insufficient and/or illegible information.

9.  I/We accept the fact that if in any manner I/we do not comply with the requirements of the applicable export programme (s), which include interception rate of quarantine pests, could result in expulsion from further participation in the present season.

10.  I/We accept the opinion, protocol, Good Agricultural Practices (GAP), inspections and requirements of the South African National Plant Protection Organisation (DoA: PH, APIS) as final and in accordance with the applicable export programme (s).

2.  I herewith, in my capacity as producer, declare that all the information completed in this document is correct, and I will take full responsibility for, and comply with, all the requirements as prescribed in the respective export protocols for the export of fresh fruit to the country (countries) as indicated under sections 3 & 4.

APPENDIX A

TO BE COMPLETED BY ONLY PRODUCERS INTENDING TO EXPORT TABLE GRAPES TO ISRAEL

ORCHARD SANITATION OF LISTED QUARANTINE ORGANISMS FOR TABLE GRAPE EXPORTS TO ISRAEL

It is important to take note of the following requirements as set out in the protocol:

Plantex must be used for the control of the banded fruit weevil (BFV) in the Hex River Valley. It must not be applied directly onto the stems of vines younger than 3 years old. It should be applied during September before the banded fruit weevil moves upward into the vines. The Plantex band should be positioned about 25 cm below the branching of the main stem.

·  The grapes must originate from vineyards that are free from Aleurocanthus woglumi and from areas free from citrus groves with at least 500m between the edge of the vineyard and the citrus groves.

·  The grapes must originate from vineyards that have been free from Quadraspidiotus perniciosus during the last two growing seasons.

ORCHARD SANITATION:

Please provide the following detail regarding orchard sanitation of the listed quarantine organisms according to the guidelines provided below:

§  Blank spaces will results in rejection of application.

§  The comment “Do not occur in the area of production” will results in rejection of application

§  Inspectors of the Department of Agriculture (DoA) will verify information when visiting production units during the season.

1. What monitoring system is used?

a.  Trapping – details on the kind of traps used. (Pheromone traps are required by the protocol for Ceratitis rosa).

b.  Surveys – details of survey (e.g. visual inspections of trees/vines or fruit/leaves/organisms sampled for inspection with microscope).

a.  Traps are placed per hectare?

b.  Trees/vines per hectare are surveyed?

3. According to which criteria (random selection or specific guidelines –details) are trees/vines selected for:

a. Placement of traps?

b. Survey?

4. Frequency of monitoring (e.g. once a week)

6. a. In detail, please summarize the results of the pheromone trapping for the period 1 July – 1 October preceding

the export season

b. In detail, please summarize the results of the surveys conducted for Quadraspidiotus perniciosus during the preceding 2 growing seasons.

7. At what stage (threshold) are control measures applied to control Ceratitis rosa? (It is required that no C. rosa was trapped 3 months prior to export).

8. What control measures (chemical, biological and cultural) have been applied in approximately the past 5 years? Do not supply records but provide a summary of these measures (e.g. chemical: product name and rate of application)

9. What control measures are available and considered for application should it be deemed necessary?
