Official Scholarship Entry Form

Introduction

The SASC Scholarship is open to any graduating senior attending the SASC Conference each year. The scholarship is funded by contributions made by SASC member schools. The scholarship recipient will be selected by a committee, which will meet during the conference. The decision of the committee will be final. Regular member schools may have one applicant and diamond member schools may have two.

Entrance Requirements

§  Applicant must be a graduating senior delegate in attendance at the SASC Conference.

§  Applicant must complete and submit four copies of the SASC Official Scholarship Entry Form and one electronic copy of the Official Scholarship Entry Form.

§  Applicant must submit four copies of an official transcript and one electronic unofficial copy of the same transcript. An official transcript will be an original document, signed and dated by a school official. It should be in a closed envelope and have a school seal. §  Applicant must submit four copies and one electronic copy of a TYPED statement describing what the scholarship means to him/her and how he/she plans to use it.

§  The application must include four copies and one electronic copy of a TYPED statement from a nominating adult (teacher, advisor, counselor, principal, employer, etc…) explaining why the applicant deserves the scholarship.

§  All materials, including the completed entry form, transcripts, and the two typed statements/letters, must be postmarked and mailed no later than Sept. 16th. The electronic copies should be sent to and received by the same postmark deadline. Please mail to:
