The City is providing Appendix ‘D’ in Microsoft word format for ease of completion. The tables below in the Microsoft Word format will be expandable.

All blank spaces in the Form of Tender should be completed. Any failure by a Bidder to complete the Tender Form may result in preference being given to competing Bidders. If your RESPONSE is considered to be lengthy, Respondents can provide an “initial” RESPONSE and then provide the details as an Appendix referencing the Section and Requirement number (i.e. Section 1.0, Requirement 1.1).

Section 1.0Bidders Experience and Experience and Qualifications of Personnel assigned to the Work

1.1 / Provide a list of five (5) previous installations completed in the last three (3) years, preferably within Canada of the Products offered.

Section 2.0Proposed Schedule to meet completion date September 1, 2015

2.1 / Bidders are to provide a proposed schedule that must meet the final acceptance completion date of September 1, 2015. The construction schedule should include tasks plus their dependencies and timelines from issuance of award of Contract. The City anticipates to award the Contract June 1, 2015.

Section 4.0Financial

4.1All pricing is in Canadian dollars and is an inclusive price for the entire scope of work which includes, all necessary permits to complete the work, including, but not limited to all materials and products, design, fabrication, plant, labour, tools, equipment, travel, transportation, freight (F.O.B. Merle Logan Field, 2300 Bowen Road, Nanaimo, B.C.), unloading, customs clearance, duties, all applicable taxes (except for GST), labour, supervision, temporary power, lighting, including all components and any ancillary items necessary to complete the project to be fully operational to the satisfaction of the City. Delivery of all the materials and equipment shall be included in the price freight prepaid FOB to the City project site location.

4.3Optional Pricing
