REQUIREMENTS FOR FULL MEMBERSHIP/ELDERS ORDERS 2014

GENERAL INFORMATION: All of these forms have been or will be emailed to you and can be completed electronically. If you are missing anything, please contact me. Also, the Board expectation in accordance with denominational standards that attention will be given to inclusive language in both the written responses and during the interview.

The following information must be filed with the Registrar of the Board of Ministry before the Board can consider your request for an interview and Full Membership, and to its subsequent recommendation to the Clergy Session of the Annual Conference (The Book of Discipline, 2012¶335.)

ALL MATERIALS MUST BE IN YOUR FILE by the due dates shown below. ALL ITEMS MUST BE IN YOUR FILE BEFORE YOUR INTERVIEW WITH THE BOARD CAN PROCEED. Retain a copy of everything you submit to the Board.

THE FOLLOWING ITEMS ARE ALREADY IN YOUR FILE AS INDICATED ON YOUR CHECKLIST:

# 1 Provisional Membership Journal Record

# 2 District Superintendent Annual Report

In order to receive Full Membership and Elder’sor Deacon's Orders a person must serve a minimum of two years full-timeunder appointment and under the supervision of a District Superintendent in this Annual Conference. (Conference Policy, Journal, 1990, pg. 154.)

#3Counseling Mentor Report

These are due annually by December 10. Check with your mentor if these are missing.

#4Final Seminary Transcript (Form 170) (This form is included if I do not already have it)

  • Complete seminary transcript directly from the seminaryto the Registrar (copies unacceptable)
  • “Seminary Information Form “ (Form 170) from PM/C packet certifying graduation from anaccredited seminary (Discipline, 2012 ¶335.3b
  • “Seminary Information Form”(Form 170) certifying you have successfully completedcourses on United Methodist history, doctrine and polity or evidence that the requirements have been met by completing “an independent study program provided and administered by the Division of Ordained Ministry.” (Discipline, 2012,¶ 335.3d) All verifications must cometo the Board of Ministry Registrar directly from theagency. Copies are unacceptable. [Verifications of thecompleted history, doctrine and polity must be received before the interview date, if not completed by seminarygraduation)

Be sure to have Form 410 signed by your District Superintendent. Be sure to indicate the waiver of your choice and sign. I will email you a copy of this form in Word format to make this easier on all of us. In addition, if possible, please email this list to me formatted in Word as Avery #5160 labels. This saves me a good bit of deciphering handwriting and retyping. If this is NOT possible for some reason, please type the names legibly and either mail or email to me. I will mail the forms to the laity and these will be returned directly to me.

Completed written answers (1 copy) to the questions found in the Book of Discipline, 2012 ¶335.7. (Form 400)

# 13Worship Materials (combination of email and mailing)

  • Submit on DVD (10 copies): the recording of an entire worship service conducted in

the setting of your appointment including a sermonpreached from one, or more, of the attached selections

of the Revised Common Lectionary. (Mail these to me; they will not be returned.)

  • Send a hard copy as well as an emailof the manuscript of the sermon, double-spaced and typed.
  • 10 copies of the bulletin from the above service (Mail these)

# 14Teaching Plan

Submit a plan and outline for teaching a book or books of the Bible (Discipline, 2012 ¶335.6) including goals and objectives of the study and outline of the content. This is to be a study that you have led.

Specifically the Board requires the following:

a. Present a statement as to what is the purpose and goal for the Bible study and how it reflects a connection between the selected book of the Bible and the Christian life.

b. Provide an outline of the scripture text and an overall teaching plan.

c. File the curriculum (lesson plan) for one 60-90 minute Bible Study session (i.e. methodology, media presentation, background information, questions,discussion points, etc.) for how you taught one particular session.

Mail me one hard copy of everything, but also email me one copy for distribution to the team.

#15Ordination Project

See the attached guidelines. These projects will be presented by you on the day of the interview.

About your interview with the Board: Your interview will focus on your readiness for and effectiveness

in ministry. Your experience in the provisional period and the materials in your file, as well as some

hypothetical pastoral situations, will form the basis for your interview.
