APPENDIX A:TENDER OFFER FORM

Bidders are to provide their response directly onto Appendix A. Bidders are asked to provide a response to all items. Bidder(s) must not alter any portion of Appendix A,except for adding the information requested. To do so will invalidate the submission of its Tender.

SECTION A:BIDDER INFORMATION

Please indicate the Equipmentowned and intended to use for the Work. Include description of Equipment (i.e. truck with snow blade, backhoe, etc, salting and sanding, etc.), Year, Make and Model.
Does your company have a salt/sanding spreader? State Yes or No.
Describe your commitment to the City to provide the Parking Lot Snow Clearing and Ice Control Services.
Previous Experience. Please state three (3) references for the last three years you have provided services to that are identical or similar to the requirements of this RFT. Information to include is business name, contact name, phone number, Contract date, services provided.
Contractor’s Personnel. State the training, qualifications and experience of the personnel that will undertake the work.
Sub-contracting. Bidders are required to submit a list of potential sub-contractors for approval by the City. The Contractor is responsible for supervising and coordinating all services delegated to the sub-contractor and the Contractor agrees if awarded a Contract; the Contractor will bind every sub-contractor by the terms and conditions of the Contract between the Contractor and the City. Only those sub-contractors listed in this section may be utilized in this Contract for the provision of any Services, unless prior approval is granted by the City.

SECTION B:PRICING

Hourly rates quoted are in Canadian Funds, excluding HST, and includes all charges for the Equipment and operator, labour charges, fuel (including surcharges), tools, truck charge per call, move charges, travel charges, maintenance, transportation, loading & unloading, supervision, permits, insurances and licenses, overhead and profit and all other charges and requirements necessary for the performance and completion of the Parking Lot Snow Clearing and Ice Control Services.

Hourly rate calculation for invoicing is to be based on running time from when the Equipment begins clearing a specific facility site to when the Equipment is no longer running for the purpose of clearing snow and applying salt or sand.

Hourly rates shall apply to time-on site only and shall incorporate all amounts provided for possible overtime seven days per week (Monday to Sunday).

SECTION D:ACCEPTANCE OF RFT TERMS AND CONDTIONS

In order to be entitled to consideration, Appendix A, SectionDAcceptance of RFT Terms and Conditions must be signed by a person authorized to sign on behalf of the Contractor and to bind the Bidder to statements made in response to this Request for Tender.

Through submission of this Tender I/We agree to all of the terms and conditions of the Request for Tender and agree that any inconsistent provisions in our Tender will be as if not written and do not exist. I/We have carefully read and examined the Request for Tender documents, and have made such inquiries and conducted such other investigations as were prudent and reasonable in preparing the Tender. Through submission of this Tender I/we agree to all the terms and conditions of the Request for Tender and agree that that any inconsistent provisions in our Proposal will be as if not written and do not exist.

Should I/We be identified as the successful Bidder I/We are willing to enter into an Agreement with the City of Nanaimo on the terms and conditions contained in Appendix I Form of Agreement.
