6) BRIEF RESUME OF INTENDED WORK:

6.1) Need for the study:-

Ayurveda aims at preventive and curative aspects with various therapeutic measures by means of samshodhana and Samshamana. Among shodhana therapy, the basti karma is claimed as the main radical treatment seeking the absolute cure of disease by eliminating its root cause 1a.Yapanbasti is a type of asthapana basti which is synonym of niruha basti,it is suitable in all seasons1b. Yapana basti is considered to be having particular action on shukra dhatu.

In Āyurveda, sāra of all dhātus is considered as Seventh dhātu i.e. Śhukra. Ksheena shukra is a condition in which there is qualitative and quantitative decrease of shukra charecterised by duarbalya, mukhashosha, pandutva, shrama, klaibya, shukraavisarga, leading to infertility. In Upanishads also mentioned the Putra asina is one of the primary desires of mankind.Charak has described that children (PutraPrapti) are responsible for Dharma (way to reach heaven or divine), Artha (means to earn livelihood), Preeti (way for harmony), Yasha(fame) 1c and this all depend upon Shukra.

Condition Ksheena shukra can be correlated with Oligospermia By definition, the Oligospermia is characterized by the lowered sperm count which is less than 15 million/ml.6

Statistics available over the past 20years reveal that, In approximately 30% of case of infertility, the pathology is found in male alone. In contrast to this, 20% of infertility is due to abnormality present in both man and women. Logically this reveals incidence of 50% of male factor in infertile couples in general. The WHO statistical analysis drawn from 7273 couples revealed that Male factor accounts for 24% of infertility cases, female and male factor accounts 24% further, only female factors were identified in 41% cases of infertility. Also non demonstrable factors of infertility were accounted in remaining 11% of couples.7

In conventional system of medicine management of this condition is been reached to most advance technique i.e. MART(Medical assisted reproductive technique) like Artificial Insemination, In-vitro Fertilization, Intra cytoplasmic sperm injection which are highly expensive and results are not satisfactory.In Ayurvedic Classics, it is explained that Shukra doshas are specially managed by basti karma .1d

Different types of effective Vajikarana basti’s are explained in the management of Ksheena Shukra. Most of these are yet to be explored. Many research works have been done on oligospermia but only few concentrated on standardization of procedure, but those studies were unable to draw a proper conclusion. So an attempt is made to Standardize the procedure of mixing basti dravya prepared from two different methods i.e. khalwa (mechanical) and mixer (instrumental) method and analyse the efficacy of baladi Yapana basti in Ksheena Shukra.w.s.r.to Oligospermia

The management of ksheena shukra is explained in detail in brihatrayees.

In ksheena shukra there will be pain in phallus & testicles associated with delayed seminal discharge or very less in quantity

d)Sushrutha also considered Ksheena Shukra as one among the 8 Shukra Dushti.and

mentioned in Sutrasthana.3 (A.S. Su. 19/9),

6.3) Objective of study:

Aims & Objectives

1)To standardize the Preparation of basti dravya with khalwa (Method I) and

mixer method (Method II).

2) To study the efficacy of Baladi yapanabasti in the patient of ksheena shukra by

Method I and by Method II.

3)  To compare the efficacy of Baladi yapanabasti in the patient of ksheena shukra from both above methods.

7) MATERIALS AND METHODS

7.1) Source of data:

30 patients of ksheena shukra (oligospermia) will be selected from OPD IPD of

SRI JAYA CHAMRAJENDRA INSTITUTE OF INDIAN MEDICINE Hospital Bangalore.

7.2) Method of data collection:

For the present study, total 30 patients will be selected from the O.P.D.& I.P.D of

SRI JAYA CHAMRAJENDRA INSTITUTE OF INDIAN MEDICINE Hospital,

Bangalore.

•  The patients will be selected irrespective of their religion, socio economic status , fully satisfying the clinical criteria for diagnosis of Ksheena Shukra (Oligspermia)

•  A case proforma will be specially designed duly filled with history taking, clinical examination and laboratory investigations.

•  The parameters of signs and symptoms will be scored as per the proforma.

Diagnostic criteria:

Ø  Mainly based on the signs and symptoms of ksheena shukra and Semen analysis.

Inclusion criteria:

•  Age between 25-50 years

•  A male complaining of infertility with minimum of 1 year of married life having unprotected sex.

•  The patients having classical signs and sympomts of Ksheena Shukra.

•  The WHO criteria (2010) for semenogram i.e. sperm count < 15 million/ ml

•  Patient fit for basti karma

Exclusion criteria:

•  Patients with Absolute Azoospermia.

•  Patient with structural abnormality of gonads.

•  Patients with major systemic/infectious disorders like Tuberculosis, AIDS, and STD

Which interfere in course of treatment .

•  STUDY DESIGN:

It will be a comparative clinical study with pre and post test design

30 patients of Ksheena shukra (oligospermia) who fulfils the inclusion criteria will be

selected and randomly assigned into following 2 groups each comprising of 15 patients.

Group – A

Patients of this group will be given a course of baladi yapan Basti

Dravya Prepared in KhalwaYantra------(Method I)

Group – B

Patients of this group will be given a course of baladi yapan Basti

Dravya Prepared in Mixer ______(Method II)

In both method after preparation of basti dravya,samples are taken for Analysis

in laboratory.

The details of the basti administrative process is as follows.

Patients of both groups are administered Kala Basti1course as mentioned in classics.

During pilot study it is observed that 12 prasrutha dosage of basti mentioned in charaka Samhitha was difficult to administer,so the mamdhyama matra mentioned in sharangadhara Samhitha is taken for present study. 5

DURATION & FOLLOW UP:

After the completion of basti karma, patient reviewed after one month.

STUDY DURATION:50 days

Assessment criteria:

Ø  Assessment of Procedural study will be done to know the particle size of final Basti Colloid, pH, Viscosity,Concentration in both the method of preparation by Khalwa (motor pestle) and Mixer.

Ø  Assessment of clinical study will be done based on following Subjective and Objective parameters.

SUBJECTIVE PARAMETERS

-Classical laxanas of ksheen shukra.

1.Duarbalya

2.vrashana and medhra vedana

3.Shrama

4.Klaibya

5.Shukraavisarga/alpapraseka

-sexual health parameters like sexual desire, erection, rigidity and orgasm. The

Scoring system developed by Mehra and Singh (1995) will be adopted .

OBJECTIVE PARAMETERS

-Semen Analysis :

(volume, count, pH,viscosity,motility)

TEST OF SIGNIFICANCE

Assessment of the condition will be done based on the detailed proforma adopting standard methods of scoring of subjective parameter which will be analyzed statistically by using Chi-square and Student`s ‘t’ test.

In both the groups assessment is made on 0,18th ,50th day, and conclusion will be drawn
