Instructions to fill in the Form

A)Project Information

1.Name of the project and location

Indicate the name of the project in the Project documentand also mention the actual locations of the project.

2.Sector/Sub-Sector

Indicate Sector & Sub-Sector in which the project falls.

3.Proposal Submitted by

Indicate complete details of the chamber/ Association/ Ministry / Agency / entity who submitted the project proposal for accessing funds through EDF.

4.Date of Approval

Mention date of approval of the project form Technical Committee (if it was referred to technical committee) and from EDF Board

5.Executing Agency

Indicate the details of the Organization implementing the project.

6.Agency for Operation & Maintenance after Completion

Indicate the name and address of the Agency/Organization that will be responsible for the maintenance / operation of the project after the completion to ensure sustainability of the activity.

7.Completion Period

Indicate actual dates of commencement and completion.

8.Objectives achieved through the project

Capital cost means cost of all those goods & services which are needed to complete and operationalize the project. (You need to mention all the costs incurred so far)

Revenue cost means cost of all those goods & services which are needed to for full operation and functioning of the project.

D)Physical Progress

Provide list of all activities performed so far. This list should be provided in the order in which activities were started. Cost incurred for completion of each activity should be provided. Its start time and finish time in months be provided.

Indicate the details and type of services being rendered and income being generated.

Indicate the details and type of exports generated.
