POST:Case Manager (NOMS)

HOURS:Full Time (35 hrs per week)

REPORTING TO: CF03 Contract Team Leader

SALARY: £20,000per annum

DURATION Permanent

LOCATION: HMP Eastwood Park/A range of Community settings

PENSION:Auto Enrolment Pension in force (6% employer contribution)

PURPOSE

The Case Manager will be providing specialist advice and guidance to a caseload of women clientswho present with a range of complex needs andrelated barriers to social inclusion on the CFO3 contract.

Key to this role is the ability to motivate clients to overcome barriers; working with them to reduce reoffending and reintegrate back into the community through employment and learning.

The Case Manager will be working predominantlywith clientsin custody within HMP Eastwood Park (70% of caseload). Support will also take place within the community (30% of caseload).

Service Delivery Tasks

The role is to:

  • Manage a caseload of female clients with complex needs; providing specialist support to overcome barriers to social inclusion
  • Provide specialist assessment, advice and guidance and signposting to internal and external training and support
  • Work closely with the prime provider case management team and other partners; creating a “village” support network for each client
  • Operate with a charitable person-centred approach whilst demonstrating commercial acumen
  • Manage and maintain a caseload of up to 35 clients at any one time, whilst remaining compliant and providing the highest quality service at all times
  • Build relationships and work collaboratively with partner organisations, providing clear feedback and reports
  • Support clients to move towards employment, short courses, vocational training, community activities and other supportive measures
  • Encourage and facilitate client autonomy, personal choice, independence and responsibility
  • Maintain a professional relationship with clients on the programme and with other staff and partners, paying particular attention to confidentiality and the maintenance of professional boundaries
  • Work flexibly as required by the needs of contract and clients, which will include travelling (it may also include some unsociable hours)
  • Ensure compliance with all contractual obligations and to have a working knowledge of Nelson Trusts policies and proceduresand ensure they are adhered to (including Health and Safety, Safeguarding and Equality & Diversity policies)
  • Attend training and development sessions and all relevant meetings as and when directed or identified.
  • Maintain accurate records of all financial transactions using the forms approved by Nelson Trust and within the agreed budget
  • Maintain administrative systems which record the progress of clients and keep accurate and complete records of client action plans
  • Prepare all necessary reports and information for internal or external audits, inspections and assessments contributing toward evaluation and monitoring of services.
  • Develop own practice by maintaining and improving knowledge of relevant work practices.
  • To maintain close contact with the line manager to ensure effective communication regarding any challenges or improvements to the service making use of line management and supervision as provided.

Notes

The post holder will be required to undergo a satisfactory enhanced DBS Check and NOMS Security Clearance.

The post holder must hold a valid driving licence and have access to a car.

In order to deliver services effectively, a degree of flexibility is needed, and the post holder may be required to perform work which is not specifically referred to above. Such duties however, should not normally exceed those expected of an employee at that grade.
