Patient Policies

The following has been compiled to help better serve your medical needs.

Please read through this information and keep handy for future reference.

The Physicians at this clinicdo bill Medicare, but do not accept assignment.

If you have questions regarding this, please call Patient Services.

Phone hoursare 8:30 am-12:00 noon and 1:30 to 4:30 pm Monday thru Friday. You are more likely to receive a prompt response to your inquiry if you call early in the day.

For urgent, after hours advice, you can call the office to reach the on-call provider.Calls to the above numbers are transferred to the answering service after hours, the on call provider will be paged to return your call. For life threatening emergencies, Call 911.

All labs must be scheduled. Laboratory hours are 8:30-4:30 Monday

7:30-4:30 Tuesday thru Friday

Doctors or Medical Assistants will call with abnormal results when results are in.

Hours for Medical Assistant visits are the same as the phone hours listed above.

Please request all medication refills from your pharmacy three business days before you run out. On call physicians maynot refill prescriptions after hours.

Co-pays are due at the time of visit; this includes any applicable co-pays for protimes (blood work) and walk in blood pressure checks.

You need to have your insurance card at each visit.

If calling to cancel a doctor’s appointment, you must give 24-hours notice. We reserve the right to bill you for any no shows and dismiss patients for failure to keep appointments.

You are responsible for the cost of any test and/or services provided that are not covered by insurance.

Past due accounts are subject to interest charges.

Cash paying patients must speak to the Patient Services prior to each visit.

Patients paying in full at the time of the visit will get a 10% discount (including any labs).

If the patient is unable to pay in full, payment arrangements will need to be made.

Arrangements of 50% payment at time of visit with balance due at next statement are usual. Patients failing to keep to the payment arrangements will invalidate the agreement and balance will bedue and payable immediately.

All appointments with our specialists must be initiated by your Primary Care Provider.

If your insurance requires a referral, it is your responsibility to ensure that the referral is in place before your appointment or lab work is performed.
