How to Register and the Practice Boundary

Wake Green Surgery is regrettably currently not accepting new patients. The only exception to this is family members of current registered patients living in the same household. This is to ensure the surgery sustains a workload that is safe and manageable. NHS England is aware of this decision.

The practice area covered is most of B13 and adjacent areas of B12 and B14. Proof of address is desirable(but not essential) when registering. Please speak to a member of the reception team to register.

It often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a registration form, available at reception. Please note a separate form for each family member is required. Medical treatment is available from the date of registration. Whilst we will allocate to you a named GP, you have freedom to choose which GP you see at the practice by speaking to our staff when you call/visit or alternative selecting your GP of choice when booking appointments online.

Out of Hours

Out of hours calls should be made to 111 (free call) and your call will be diverted to our Out of Hours Service (provided by Primecare) where a fully trained healthcare worker will give you advice, refer you to a Doctor and/or organise a home visit. This service will be used only overnight, at weekends and for public holidays. The CCG commissions this service for its patients.

Open daily between 08:00-20:00. No appointment necessary. For urgent care needs.

Appointments

A range of consultations are available including: daily minor illness surgeries, routine appointments, phone consultations and commuter surgeries. Appointments are released on a daily basis up to two weeks in advance and can be booked in person, over the phone or by pre-registering for our online booking service at: Please be responsible about cancelling unwanted appointments so they are not wasted. We will treat you with dignity, respect and an awareness of cultural sensitivities.

Home Visit

Available to those who are house bound. The duty doctor will call to discuss your needs so that the most appropriate solution can be found. If you feel that you need a home visit please try and phone the surgery before midday, this enables us to allocate the most appropriate clinician.

Repeat Prescriptions

Please allow 2 working days for all repeat prescriptions. We offer an Electronic Prescription Service (EPS) which allows your prescription to be sent directly to a Pharmacy of your choice (i.e nearer to home or work. Please speak to reception or your GP if you would like to use this service.

Accessible Information Standard

We want to make sure that we are communicating with you in a way that is easy for you and that you can understand. As a Practice, we are currently working towards the Standard and identifying those patients with information and communication needs.

We will do our best to ensure that your needs are met and with your permission record this in your file and share the information with other NHS and adult social care providers.

We will ask new registered patients about their specific needs on our registration form.

For existing patients, our staff will routinely ask when you visit. Please help us to communicate effectively with you by making us aware of your specific needs.

Access

The practice is Disability Discrimination Act compliant with good access for all. Two disabled parking bays are available at the front of the building with ramp access to the door. There is a disabled access point at the bottom of the ramp if you need assistance and we have a portable hearing loop if required. Interpreter services are available for consultations – please discuss this with reception team.


Data Protection and Confidentiality

We take the confidentiality of your personal and medicalinformation seriously. The practice is registered with the Data Protection Act and bound by the terms of this.Your information is stored securely within our clinical system and is protected by NHS ITSafeguards. All members of staff have signed aconfidentiality agreement.

Access to Records

In accordance with the Data Protection Act 1990 and Access to Health Records Act, patients my request to see their medical records. Such requests should be made through the practice manager and may be subject to an administration charge. No information will be released without the patient’s consent unless we are legally obliged to do so.

Compliments, Comments, Concerns and Complaints

Your experience matters to us and we want to hear from you, your input gives us valuable information aboutwhat we do well and what we could do better.

If you have a complaint or a concern about the service you received from any member of staff working at the practice please let us know. We hope that any concerns can be resolved easily and quickly at the time they arise. If you wish to make a formal complaint please note Wake Green Surgery operates a practice complaints procedure as part of a NHS system for dealing with complaints. Please pick up a ‘Complaints Procedure’ leaflet from the surgery or see our website. If you make a complaint it is practice policy to ensure you are not discriminated against or subjected to any negative effect on your care, treatment or support.

Teaching Practice

We are a teaching practice with the West Midlands Deanery. Trainees are fully qualified doctors who have chosen General Practice as a career. They are always supported by a fully qualified GP who is available to offer assistance whenever needed.

The Patient Participation Group

Comprises of a group of patients and carers who meet with us on a regular basis. The group is there to help us understand the patient’s view and we also use the group for advice or to comment on new ideas we may have. Please see the PPG board in the waiting room for more details.


Patients’ rights and responsibilities to your GP Practice

  • Every UK resident has the right to be registered with a GP Practice.
  • Your GP Practice is your gateway to all other health services.
  • You can see a GP within 48 hours if you want to be seen that quickly.
  • You can book appointments in advance so that the appointment is as convenient as possible for you.
  • Your GP Practice may offer a telephone discussion with a GP or Nurse if that is convenient to you.
  • You can have at least 10 minutes with your GP / Practice Nurse to fully discuss any health issues.
  • Your GP Practice can offer you general health advice or advice / treatment in relation to smoking, diet, exercise, alcohol consumption, drug usage, etc. They are not just there for when you are ill.
  • Use your NHS services wisely, if you have an appointment booked but then don’t need it or can’t make it, then please remember to contact the surgery and cancel it. Many hours of doctors' time is wasted by patients who fail to attend for their appointments, and this means other patients cannot be seen or have to wait longer.
  • Everyone will be treated with dignity and respect – patients and staff alike.
  • If you change your address or telephone number, please inform your GP Practice so that they have accurate information if they need to contact you.

Zero tolerance policy with regard to violence and abuse

The NHS operates a zero tolerance policy with regard to violence and abuse. The practice has the right to remove violent patients from the list with immediate effect. This is in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and a record in the patient’s medical records the fact of the removal and the circumstances leading to it.

We aim to provide our patients with the highest possible standards of medical care and to treat our patients with respect and courtesy.
