Declaration:
By supporting Plymouth Dementia Action Alliance you will be working to achieve the aims of the National Dementia Action Alliance and making a positive difference to the lives of local people and their carers living with dementia.
The National Dementia Declaration lists seven outcomes that the Dementia Action Alliance are seeking to achieve for people with dementia and their carers. These are:
●I have personal choice and control or influence over decisions about me.
●I know that services are designed around me and my needs.
●I have support that helps me live my life.
●I have the knowledge and know-how to get what I need.
●I live in an enabling and supportive environment where I feel valued.
●I have a sense of belonging and of being a valued part of the family, community and civic life.
●I know there is research going on which delivers a better life for me now and hope for the future.

- How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
This could include:
●Training and awareness for all staff (if so how many?),
●Review of facilities so people with dementia can access services more easily (making sure signs, leaflets or brochures are clear and appropriately placed).
●A review practices and policies which may affect people with dementia, and implementing innovative solutions to the barriers to access services.
●Awareness raising of dementia for the wider public via company publications.
●Human Resources policy review to ensure support for carers within your organisation.

This could include lack of knowledge amongst staff, limited time or resources, limited budget, not a primary purpose of the business so not a priority or any other barriers to success. (150 words)
