This post is fixed term for 12 months and is available full or part time. The post is based mainly at the Royal Hospital for Sick Children, Edinburgh, a tertiary children’s hospital providing comprehensive secondary and tertiary level care for the children and young people in Edinburgh and SE Scotland. You will join a team of four other established Consultants who deliver paediatric rheumatology care for children around the whole of Scotland as part of a managed clinical network, SPARN. You will also be expected to contribute to an out of hour’s telephone on call rota. Applicants should have specialist training in Paediatric Rheumatology (National Grid or equivalent).

JOB DESCRIPTION

This post of Locum Consultant in Paediatric Rheumatology is a fixed term replacement post to cover a one year period of leave. The post is available full time or less than full time.

The post holder will work with closely with one other Consultant Paediatric Rheumatologist to deliver the Paediatric Rheumatology service in Lothian and will be based at the Royal Hospital for Sick Children, Edinburgh.

The appointee will work as part of a team of five Consultants (4 NHS Consultants and 1 Senior Lecturer in Paediatric Rheumatology) working within SPARN. This team supports Paediatric Rheumatology Services around Scotland, with tertiary services in Glasgow and Edinburgh and network clinics in all health board areas of Scotland.

NHS Lothian

Edinburgh is the capital city of Scotland and has a population of approximately 500,000. NHS Lothian serves a population of 800,000 and was created on 1 April 2004 following dissolution of 3 Trusts; Lothian University Hospitals Trust, Lothian Primary Care Trust and West Lothian Healthcare Trust.

The University Hospitals Division of NHS Lothian provides a full range of secondary clinical services to the populations of Edinburgh, Midlothian, East Lothian and West Lothian and is the main tertiary centre for the South-East of Scotland, a population of c. 1.9 million people The Division is one of the major teaching centres in the United Kingdom.

Royal Hospital for Sick Children (RHSCE)

The Royal Hospital for Sick Children, Edinburgh (RHSC E) is the main Paediatric teaching hospital for the South-East of Scotland. This 144 bedded hospital provides general paediatric care to the children of Edinburgh and surrounding areas, and specialist services on both a local and national level. Tertiary services provided include intensive care, gastroenterology, hepatology and nutrition, endocrinology, rheumatology, respiratory medicine, cardiology, nephrology, neurology, neurosurgery, inherited metabolic disease, oncology, neonatal surgery, plastic surgery, orthopaedic surgery, urology and general surgery. In addition, RHSCE is the designated national centre for spinal scoliosis surgery.

The hospital accommodation includes a dedicated paediatric outpatient facility, medical and surgical day case units, theatres, a critical care unit comprising an 8 bedded PICU and 6 bedded High Dependency Unit.

There is an excellent library facility and a modern lecture theatre with a full range of audio-visual equipment.

All services are supported by comprehensive radiology, neurophysiology and laboratory services. The local radiology department provides on site Magnetic Resonance Imaging, CT Scanning, nuclear scanning and ultrasound. On site laboratories provide biochemistry, haematology, pathology and neuropathology services. Comprehensive pharmacy services are available on site.

RHSCE has therapy departments providing occupational therapy, physiotherapy, speech and language therapy, and dietetics.

The hospital is currently situated in a quiet residential area close to the centre of Edinburgh. It is approximately 3 miles from the new Royal Infirmary and University of Edinburgh Medical School. It is anticipated that it will move to its new location on the Little France site in the first half of 2018.

The University of Edinburgh and its Medical School have an outstanding reputation for teaching and research. RHSC provides the major paediatric undergraduate training centre for the South East of Scotland, housing both the School of Community Paediatrics and the University Department of Child Life and Health. In addition, the hospital has very strong links with the University’s Department of Neonatology.

RHSC is managed as part of the Women and Children’s Clinical Management Team. Dr Edward Doyle is the Clinical Director,

NHS Lothian is currently re providing RHSC Edinburgh on the Little France site, co-locating with the Royal Infirmary of Edinburgh, the University Medical School, and a privately-funded medical research park. The building is nearing completion and it is anticipated that the move to the new hospital will occur during the period of this locum posts, most likely in the early summer of 2018.

Paediatric rheumatology service

Since 2005 the paediatric rheumatology service in Edinburgh has been delivered by the consultant paediatric rheumatology team based in Glasgow under a service level agreement with NHS Lothian. Two Consultants have jointly delivered the service, one with a clinical base in Edinburgh.

The service in Edinburgh has continued to grow and develop, with referrals from general practitioners, paediatricians and adult rheumatologists in Lothian and surrounding areas extending to the Borders, Dundee and Fife. In 2016 over 240 new patients with a variety of diagnoses were seen in the service.

Network clinics, as part of the Scottish Network for Paediatric and Adolescent Rheumatology, are delivered by the Edinburgh based Consultants in Aberdeen, Fife, Borders and St John’s hospital, Livingstone.

It is envisaged that the post holder will mainly support the clinical service in RHSC, Edinburgh but will have the opportunity to become involved in some network clinics if desired.

RHSC-E has an excellent paediatric rheumatology multidisciplinary team comprising 2 WTE clinical nurse specialists, dedicated physiotherapist, part time occupational therapist and social work support. There is a physiotherapy department on site with access to hydrotherapy in a nearby special school. The hospital has an excellent pain management team and child psychiatry/psychology support. Access to Art Therapy for children with rheumatological conditions is provided by the Teapot Trust.

Clinics are held in a dedicated children’s outpatient department with a good phlebotomy service. In addition to a weekly general rheumatology clinic there are two new patient clinics per month, a monthly connective tissue diseases clinic and a monthly teenage clinic. There are regular transition clinics with one of the adult rheumatologists and joint clinics with neurology, dermatology and nephrology.

The main paediatric rheumatology clinic is run in parallel with an ophthalmology clinic providing a good service for children requiring regular eye monitoring. In addition there is a monthly uveitis clinic run jointly between rheumatology and ophthalmology providing care for children with JIA associated and idiopathic uveitis.

There is access to a monthly general anaesthetic list for joint injections as well as facilities for undertaking them with entonox in the day unit or outpatient department. The medical day unit (PIU) is accessed regularly by rheumatology patients requiring regular infusions eg biologics. In patient beds are available in general paediatric wards

The radiology department provides on site access to ultrasound, MR and CT scanning. DEXA scanning is available at the Western General Hospital.

There is a weekly multidisciplinary clinical team meeting and a fortnightly radiology meeting. There is a weekly team education session and a monthly videoconferenced SPARN CPD session. There are regular postgraduate rheumatology teaching sessions within Edinburgh and good links with adult rheumatology. There is the opportunity to attend any of the weekly CPD sessions at RHC, Glasgow

There are MCN paediatric research nurses based in Edinburgh and a dedicated on-site research facility. The unit currently contributes to the national biologics registries, the CAPS study and the UK JSLE cohort study and will be a centre for the sekukinumab study.

Scottish Paediatric and Adolescent Rheumatology Network (SPARN)

Paediatric Rheumatology services throughout Scotland now work together within a formal Managed Clinical Network within which Edinburgh has a key role.

Locally based multidisciplinary teams and paediatricians with an interest in the specialty manage the majority of children with rheumatological conditions, supported by the expert clinical teams in Glasgow and Edinburgh. There is now an established network of shared care clinics, with clinics established in all Health Board areas within Scotland.

The network holds an annual multidisciplinary network meeting and supports a variety of training and educational opportunities for staff.

The successful applicant for this new post will join four Consultant colleagues to provide expert Paediatric Rheumatology services throughout Scotland. The aim of this post will be to support the service in Edinburgh.

There will be a service level agreement with Edinburgh.

Work with one other Consultant to provide Consultant input for the paediatric rheumatology service in Edinburgh

Lead the joint injection service within RHSC (E).

Be involved in a telephone on call rota providing out of hours cover to both Glasgow and Edinburgh service

Have the opportunity to be involved in audit, research and undergraduate education depending on experience and interest.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.
