Nationwide Special Risks Volunteer Accident Insurance Application and Reporting Instructions

To apply for Nationwide Insurance for your covenant partner’s volunteer accident insurance, fill out the attached policy application form. Applications can be downloaded here:

Include the covenant partner name as you want it listed on the policy, and what mailing address you want to receive the billing notice (line 1. Billing Address). An authorized representative from your covenant partner will be required to sign the form as well.

When you fill out the original policy application, make the check payable to Nationwide Special Risks (not The Fuller Center for Housing) you will mail it and the $175 annual minimum premium fee to: The Fuller Center for Housing, 701 South MLK Jr., Blvd., Americus, Georgia 31719.

The Fuller Center for Housing will mail the application form along with your check to Nationwide. When a location number is assigned, the original policy will be sent to Fuller Center headquarters. The Fuller Center will retain a copy of the policy in your covenant partner insurance file and send the original to your covenant partner at the address listed on the application.

Reporting is due on a quarterly basis. When the quarterly report is due provide the previous three months volunteer and work day counts from the due date provided on the reporting invoice statement.

For example, if your reporting is due for the July 1 reporting cycle, you will include all volunteer counts beginning with May 1. Remember, even if the covenant partner does not have any volunteer activity for the previous three months before the reporting invoice due date, reports are required to show no volunteer activity or Nationwide will cancel your policy. These reports do not need to be provided to Fuller Center for Housing, but if you need help with the reporting don’t hesitate to ask for assistance.

The yearly premium for Nationwide is $175.00. When your covenant partner does have volunteer hours to report, please remember to subtract the premium due from your yearly $175 premium that you have already paid. It is your responsibility to keep up with your premiums paid. Nationwide does not send out invoices or statements for outstanding premiums or yearly renewals.

The Fuller Center for Housing keeps a copy of each quarterly billing notice and any past due notices or termination letters that is sent to the covenant partner. We also keep any email exchanges with a representative from a covenant partner or with Nationwide for servicing.

Helpful Tips:

  • Do not report volunteers who came from the Fuller Center US Builders Program as they have been covered under a national policy and the fees for this were subtracted from the volunteer registration fees.
  • Keep a daily calendar of any volunteers, staff or homeowners who volunteer each day.
  • Remember to send in your annual renewal payment when it is due which will be annual anniversary date the policy first became effective.
