Job Summary: Provides professional consultation and approves patient care recommendations for the Anticoagulation Clinic patients

Responsibilities and Duties:

  • Follows accepted standards of medical practice observed in the Anticoagulation Clinic to ensure proper patient care
  • Reviews the Patient Summary Sheet, pharmacists’ notes and recommendations for anticoagulation therapy prior to 6pm on the day that the recommendations are mead. Completes documentation of medical record in a timely, legible, and thorough manner to ensure the consistency of operations, compliance with established standards and for purposes of reimbursement.
  • Prescribes anticoagulation medications for outpatient therapy as appropriate.
  • Provides telephone consultations with clinic staff during hours of operation.
  • Provides coverage to respond to critical values resulted by the Hospital Laboratory prior to 6pm on days that the clinic operates.
  • Consults with other physicians to facilitate optimal patient care
  • Conducts regular feedback meetings with the major physician referrers.
  • Participates in Anticoagulation Clinic educational programs
  • Provide on site clinic coverage. One evening per week from 4:30-6:30 pm
  • Participates in development of marketing strategies to increase volume from the primary and secondary markets.
  • Provide oversight of quality improvement activities relative to the evaluation and safety of treatment managed through the clinic.
  • Consults with other physicians to facilitate optimal patient care

Requirement /Specifications:

Experience: Minimum of two years experience in primary care.

License and Certification: Current CT medical licensure. Certification as a Certified Anticoagulation Care Provider by the National Certification Board for Anticoagulation Providers preferred.

Knowledge, Skills and Abilities: General knowledge of internal medicine and family practice. Excellent verbal and communication skills. Ability to read and understand oral and written instructions, and communicate effectively in order to provide information and instruction. Ability to work collaboratively with all departments. Ability to prioritize daily work assignments. Commitment to customer service.

Supervision: Coordinates the work of others, but has no supervisory responsibility for them.
