1PURPOSE

The purpose of this work instruction is the fulfillment of legislative requirements in accordance with Section 101, Para. 3, Act No. 262/2006 Coll., Labor Code, as amended.

Contact person / An employee of Metaldyne Oslavany, spol. s.r.o., an applicant who has ordered the services from the supplier (contractor), the carrier, or is responsible for the visitor and who is responsible for him/her for the entire period of his/her activity.

These terms and conditions for contractors and contracting parties make also an agreement under Section 101 Para. 3, Act No. 262/2006 Coll., Labor Code, as amended, while the responsible person OCMF/ZCMF is charged to coordinate the work, who is authorized to act in technical matters in connection with the work/activity in question.

4.4ProcedurE

To ensure protection of health and life of the contractor's employees and to ensure a coordinated approach to the performance of the contractor's activities on the site of our plant, each contractor carrying out the work is required to inform all persons entering the premises of the company about these instructions in full,prior to them entering the company premises.

Familiarization with these terms and conditions of employees, or other persons of the contractor, must be made in writing and be ready for inspection on demand at any time.

4.4.1Workplace and Access Routes Marking

Only stay where you have your workplace contracted with us. To exclude mutual hazards, the relevant head of work must be informed and only then work can be started. Entry to other workplaces is forbidden !! Buildings, excavations, pits, boreholes, ducts, holes in the ground must be secured in accordance with valid guidelines and legislative requirements.

Permits for excavation work can only be issued by Mr. Joura, tel.:+420 602723726.

In cases where the workplace is handed over to a contractor, this must be done in writing - F-022.006 Workplace Transfer Protocol (Annex 7). The Workplace Handover Report shall be completed by Metaldyne contact person prior to commencement of work, and in case of ambiguity, the contact person will request the opinion of the other parties concerned.

5.HEALTH HAZARD OF CONTRACTORS/CONTRACTING PARTIES

5.1Written Information on Risks inMetaldyne Oslavany, spol. s r.o.

Part 5.1 will be completed by the Metaldyne contact person (applicant) in cooperation with the C EHS. This overview of risks is used to inform the contractor/contracting party of the occurrence of risks at the place of performance of his/her work in Metaldyne Oslavany, spol. s r.o. These are the risks that processes and employees of Metaldyne Oslavany, spol. s r.o. may endanger S/CP employees.

5.2Contractor’s Workers’ Access

Prior to the first entry to OCMF or ZCMF premises, each contractor/contracting party (S/CP) is obliged to provide a contact person to Metaldyne Oslavany, spol. s r.o with:

  1. Proof of liability insurance for the damage caused
  2. A list of all its employees (and subcontractors) who will enter the premises
  3. Written information on the risks (including the risks of its subcontractors) pursuant to Section 101 Para. 3 of Act No. 262/2006 Coll., Labor Code, as amended  these must also be passed on to C EHS for approval
  4. A list of all chemicals to be used in the plant (F - 022.003), including safety data sheets these must also be passed on to C EHS for approval
  5. A list of all used PPEs (F-022.004) with standard specification that will be used in the OCMF and ZCMF these must also be passed on to C EHS for approval
  6. A statement that any waste generated by it at the OCMF and ZCMF premises will be disposed of at its own expense without the use of Metaldyne Oslavany's collection means unless otherwise stipulated in the contract

Prior to the first entry to the camp, all S/CPs must check in at the reception and wait for the arrival of a Metaldyne contact person. Then they will wait for the arrival of their contact person who will carry out the training and handover of the workplace according to the requirements of SHMS PI 022.0 Safety Management of Contractors, Carriers and Visitors.

Every single day, a list of the names of all their employees (and subcontractors) moving in the OCMF and ZCMF must be checked in or presented at the reception. If it is a permanent activity (longer than 1 week), it is possible to submit a weekly list. This obligation does not apply to employment agencies that have a contractual relationship directly with Metaldyne Oslavany, spol. s r.o.

Contractors’ employees are required to move in the outdoor area of the company premises in a clothing marked with their company logo, a reflective vest (with the company logo or contractor sign, or a label with the name of the contractor) and sturdy shoes. When entering the OCMF and ZCMF production areas, S/CP staff are required to continue to use the prescribed PPE (safety shoes, long trousers and goggles in ZCMF, in OCMF also hearing protection - except for the expedition hall).

Additionally, prior to the first entry to the site, the S/CP contact person is obliged to hand over the attendance list to the contact person of Metaldyne Oslavany with a prominent acquaintance of its employees (and subcontractors) with SHMS PI 022.2 Manual for Contractual Partners (this Directive); applies to all persons entering the site. This manual is used to get to know the requirements of OSH, PO and E when moving and performing activities in OCMF and ZCMF sites.

Upon completing the above, in case of a permanent activity, the S/CP can obtain a chip card that allows them to enter the premises of the plant and production halls.

Proof of S/CP getting to know with SHMS PI 022.2 Manual for Contractual Partners (this Directive); is valid for one year from the date of signature; S/CP holding a valid appointment may, upon notice – presenting the list of persons at the reception – move in the premises without the presence of the Metaldyne contact person. For such S/CP, the Metaldyne contact person may ensure the issuance of OCMF/ZCMF entry/exit chip cards. This card will be issued at the direction of a Metaldyne OCMF and ZCMF contact person.

In addition, the Metaldyne contact person must provide training for S/C employees according to the Matrix training if the training sessions relate to their work in Metaldyne Oslavany, spol. s r.o. We recommend consulting the C EHS.

After completion of the work, it is necessary to notify the reception of leaving the plant. Please keep your work clean and safe during and after work.

5.3General Requirements

OCMF/ZCMF Records, Drawings, Papers, Production Parts, and Other Property may not be taken without permission from the production and office premises, may not be duplicated or made available to unauthorized persons. You are required to maintain confidentiality about all manufacturing and business secrets, both during and after your work.

The road traffic regulations and traffic conditions on roads (maximum permitted speed of 10 km/h on outdoor roads and 5 km/h on halls) apply. Forklifts take precedence over other means of transport.

In the winter months, some communications are not maintained. Unmaintained communications are marked with safety signs. Movement of persons on these roads is at their own risk.

5.4Requirements forPPE

Each contractor must submit a completed F-022.004 List of approved PPE to the EH&S Coordinator for approval prior to the work commencement (see Annex 8).

5.7Management of Chemical and Defective Substances

Prior to the work commencement, it is necessary to pass the list of these substances and the expected quantity to be used on the premises of the company and the safety data sheets. The contractor's responsible person will complete the list, see Annex 1. When using harmful substances in the assigned workplace, all these substances must be packed in the labeled packages that cannot be confused with the food packaging. Also, the contractor’s staff must ensure that these materials are as secure as possible from OCMF/ZCMF employees, and these materials must not be stored in such a way that OCMF/ZCMF employees can use it as well. All defective substances must be stored in an area that cannot allow them to leak.

5.8Waste Management

Any waste generated by the contractor's activities will be disposed of at its expense. There is a strict ban on storing any waste in the premises in OCMF/ZCMF containers. Each contractor has to produce sorted waste.

5.9Water Protection

When storing materials outside the company, the contractor must comply with the following rules:

a)To maintain the storage areas clean

b)Regular disposal of garbage and waste materials located in outdoor storage areas

c)Non-storage of viscous materials in the vicinity of all rainwater sewerage systems

d)Any discarded equipment must be properly cleaned

e)Plastic and metal boxes must be covered or under the roof if there is a potential risk of release of harmful substances into rainwater

f)All vessels or containers containing metal scrap must be covered or under the roof

g)Wooden pallets or wooden materials must be stacked securely and must not be stored at the walls of the building

h)Empty barrels and crates must be stored in such a way as to ensure that rainwater cannot be accumulated inside

i)Compressed technical gases must be stored in a safe place or cabinet which is lockable and must be secured against falling.

In the operation of cars or trucks in the premises of the company (OCMF/ZCMF), the contractor is responsible for all damages caused, especially for damages resulting from traffic accidents or leakage of operating fluids. In such cases, the contractor pays all the costs associated with the liquidation of these extraordinary events, including possible sanctions on the part of state or local authorities.

5.10Emergency Preparation

In the case of escape of harmful substances, the contractor's employees are obliged to ensure the disposal of leaked substances at their expense. No escaping leaking substances should be discharged into the drainage system. Any liability arising from any misconduct or negligence of the obligations is the responsibility of the contractor in its entirety. All leakages caused by contractor workers must be reported to the OCMF/ZCMF contact person who will investigate this event with the entry.

General procedure for leakage of defective substances:

a)To identify leaky substance and protect workers from exposure to this substance

b)To prevent propagation with sorption means

c)To report the leak to the OCMF/ZCMF contact person, or to the gatekeeper in case of the afternoon or night shift

d)To dispose of leakage by sorbents

e)To clean the affected area and store the used sorbent in containers designated for this purpose

f)To ensure the disposal of hazardous waste at its own expense

g)To examine the causes of leakage in the presence of OCMF/ZCMF and C EHS contact persons

h)To take measures to rectify leakage

5.11Injury and Operating Accidents

All injuries must be reported by the contractor without undue delay to its contact person. The contact person ensures participation in the EEC and conducts an investigation with a written record, Appendix No. 3. The contact person will ensure participation of C EHS and investigate the injury, and a written record will be made. SeeAnnex 2.

All operational accidents and damage to OCMF/ZCMF property will be reported by the contracting party without undue delay to its contact person. The contact person will ensure participation of C EHS and conducts an investigation,and a written record will be made. SeeAnnex No. 3

5.12State Professional Supervision

When implementing its work in the OCMF/ZCMF premises, the contracting party is obliged to tolerate the control of the state supervisory bodies (Labor Inspection, Environmental Inspectorate, Hygienic stations, etc.). In case of violation of the safety regulations, the contractor may be sanctioned by these institutions.

5.13Fire Safety

Throughout the OCMF/ZCMF premises there is a strict ban on smoking and the use of open fire. In the event that the contractor needs to do work with open fire (welding, burning, grinding, grinding) in order to carry out his services, the contractor must be authorized to do so. See Annex 4. This permit specifies in more detail the conditions under which such work may be carried out. Charged OCMF/ZCMF persons are authorized to carry out a check on compliance with fire safety requirements. In the event of non-compliance with the conditions, the contractor or his employee or subcontractor may be expelled from the premises of the company.
