POLICY AND PROCEDURE MANUAL

PREAMBLE

This Policy and Procedure Manual has been developed for the Johns Hopkins Hospital Sleep Disorders Laboratory. It has been modeled after the AASM accredited Johns Hopkins Bayview SDC. There are differences between the two centers including certain protocols, phone numbers, emergency information, and sections relating to the night technicians. Specifically in the JHHSDC, the night technicians are supplied through Sleep Services of America and policies relating to their performance are referred to the appropriate section in their Policy and Procedure Manual which should accompany this manual in the JHHSDL.

STAFF BIOGRAPHIES

Nancy Collop, MD (MEDICAL DIRECTOR)

Dr Collop graduated summa cum laude from Edinboro State College in Edinboro, PA with a BS in Biology. She went to medical school at the Pennsylvania State University College of Medicine in Hershey, PA, obtaining her MD degree in 1984. She did a categorical internal medicine internship and residency at the Medical College of Virginia in Richmond, VA, completing that program in 1987. She completed her Pulmonary and Critical Care fellowship at the University of Florida in 1990. She became a Diplomate of the American Board of Sleep Medicine in 1997 and is also board certified in Internal Medicine, Pulmonary and Critical Care Medicine. She has been the medical director of sleep laboratories at the Medical University of South Carolina (Charleston, SC), the Ralph Johnson VAMC (Charleston, SC), the Sonny Montgomery VAMC (Jackson, MS) and Sleep Solutions (a privately owned sleep laboratory in Jackson, MS). She currently serves as the President of the American Board of Sleep Medicine Board of Directors and is on the Board of Directors of the American Academy of Sleep Medicine and is a Regent-at-Large on the American College of Chest Physicians Board of Regents.

Sherry Hill ( LABORATORY COORDINATOR)

Ms. Hill trained in PSG under the guidance of Norman Schubert (RPSGT) and has worked at the Johns Hopkins Sleep Disorders Center from 1995 to the present. She is qualified to perform PSG, MSLT, and CPAP laboratory tests. In addition, she performs sleep staging, scores PSG’s, and generates CPAP reports. She obtained her RPSGT certification in 2003.

Loretta Colvin (NURSE PRACTITIONER)

Loretta Jane (Berve) Colvin is a Certified Registered Nurse Practitioner (CRNP) who has worked with the Johns Hopkins Hospital Sleep Disorders Center since April, 2006. She earned her BSN from the University of Missouri-Columbia and her MS as an Acute Care Nurse Practitioner from the University of Maryland – Baltimore. She has worked in the Division of Pulmonary and Critical Care Medicine at Johns Hopkins University since 2000. She is a member of the Nurse Practitioner Association of Maryland. Prior to obtaining her degree as a Nurse Practitioner, Ms. Colvin worked as a Critical Care Nurse, specializing in Cardiac ICU care.

Marie Bernadette Clark (Sr. Medical Office Coordinator)

Ms. Clark has worked as a Medical Office Coordinator in the Sleep Disorders Center since January 2005. Prior to joining the Sleep Center staff, Ms. Clark worked in the Division of Pulmonary and Critical Care Medicine for over a year. Prior to working at Johns Hopkins, Ms. Clark’s work experience was in administrative support for commercial companies and as director of her own child care company.

Minet Jones (Administrative Assistant)

Ms. Jones has worked in the Sleep Disorder Center since December 2005. She became and official employee of Hopkins in October 2006. Prior to becoming a permanent employee of Johns Hopkins she worked as a cashier and assistant at Giant Supermarket.

Michael Delayo, RPSGT (#84)

Sleep Services of America (SSA)

Mr. Delayo has worked in Sleep Medicine since the late 1970’s starting at the NIH and Suburban Hospital in Bethesda, Maryland Coordinating the Sleep Center and EEG Department under the direction of Wallace Mendelson, M.D., and Richard Allen, Ph.D. After 15 years, he moved to Sleep Services of America (SSA) in 1998. He received his RPSGT (#84) in 1983. Mr. Delayo now serves as the Executive Director of Education and Performance Improvement for SSA. His responsibilities include PI, Education, AASM site visit, and Technician training for over 60 Sleep Centers operated by SSA. He is active in teaching, serving as an Adjunct Professor at the CCBC Sleep Technician training course since 2001. He has developed Policy and Procedure Manuals and Technicial Training Manuals for RPSGT registration.

Rebecca Sele

Sleep Services of America (SSA)

Mrs. Rebecca Sele has worked as a Polysomnographic tech since May, 2001, with Howard Medical Services where she was trained under the supervision of Phillis Poffenbuger, RPSGT. She is qualified to perform PSG, MSLT, and CPAP laboratory tests as well as Pediatric testing. In January 2002, she joined SSA and has been assigned at the Johns Hopkins Sleep Center since January, 2003. She is preparing to sit for the BRPSGT exam.

Nadirah Bradshaw

Sleep Services of America (SSA)

Ms. Nadirah Bradshaw worked in Virginia under the supervision of Dr. Shah for three years before relocating to Maryland to work for SSA. She is currently assigned to the Johns Hopkins Sleep Center. She is qualified to perform PSG, CPAP and Pediatric testing. She is preparing to sit for the BRPSGT exam.

Meho Alic

Mr. Alic has worked as a Sleep Technician since 2004, and with SSA since August, 2005.

Sean Seymour, RPSGT (#6250)

Scoring Technologist, Sleep Services of America (SSA)

Mr. Seymour has been employed as a Sleep Technician with SSA since 2004. He received his RPSGT certification in June, 2004.

Mr. Dennis has been employed as a Sleep Technician with SSA since 2000. He received his RPSGT certification in June, 2003.

JOB DESCRIPTION: MEDICAL DIRECTOR

SLEEP DISORDERS CENTER

QUALIFICATIONS

The Medical Director of the Sleep Disorders Center will be a physician licensed in the State of Maryland with training in a medical subspecialty (pulmonary, neurology, psychiatry) and board certified by the American Board of Sleep Medicine or the American Board of Medical Specialties (CAQ in Sleep Medicine). The Medical Director will be responsible for supervising the performance and interpretation of all PSG, MSLT, MWT, CPAP and bilevel PAP tests. His/her duties include the following:

1. Quality assurance with the assistance of the Laboratory Coordinator, specifically in regard to:

a) proper equipment operation

b) equipment calibration

c) accurate data reduction

2. Responsible for patient safety.

3. Training of PSG technicians.

4. Providing timely and accurate professional interpretation of PSG, MSLT, MWT, CPAP and bilevel PAP studies.

5. Provide advice when needed to members of the medical staff regarding the appropriate use of Sleep Disorders Center services.

6. Participation in inservice education

7.  To formulate policies and procedures for Sleep Disorders Center.

8.  Expertise in providing sleep medicine consultative services.

JOB DESCRIPTION: LABORATORY COORDINATOR

SLEEP DISORDERS CENTER

JOB SUMMARY:

Provides supervision and technical support for in-patient and out-patient laboratory facilities of the Johns Hopkins Sleep Disorders Center. Responsible for the safe and efficient operation of the clinical laboratory. Performs supervision of sleep related recording protocols, analysis (scoring), data management, and report generation.

PRINCIPAL DUTIES AND RESPONSIBILITES:

Conducts services in a caring, responsive, respectful and courteous manner. Approaches work in a service driven, quality focused and cost efficient manner. Strives to achieve effective communications, teamwork and continuous improvement

Manage a budget; develop policies or procedures for Sleep Disorders Center. Write standards of performance, job descriptions, productivity, and quality control reviews. Responsible for the statistics and quality assurance monitoring of the Sleep Disorders Center. Directly responsible for maintenance of clinical records.

File management of digital and hard copy patient records.

Maintain security and backup of patient digital files and records.

Coordinates and oversees the staff scheduling and mandatory staff meetings. Responsible for attending mandatory, departmental and hospital meeting, inservices and competency programs as required by the hospital and Medical Directors.

Maintains acceptable attendance and punctuality of laboratory staff.

Arrange work schedules for employees to coincide with patient volumes, and not to incur overtime.

Responsible for securing budget approval for functions under his/her management. Applies effective cost avoidance practices in the operation of the Sleep Disorders Center.

Coordinates and performs orientation for new employees. Provides technical assistance to staff as needed

Assesses and monitors technician performance and communicates areas of performance requiring improvement.


Practices periodic maintenance performance on all capital equipment to ensure proper working conditions and a safe environment.

Notifies directors of any equipment malfunction.

SUPERVISION RECEIVED:

Works under direct supervision of Medical Director and Administrative Director.

SUPERVISION GIVEN:

Directly supervises Technician Level I.

OUALIFICATIONS:

College level training with a B.A. or B.S. degree with a major in biological sciences or the equivalent and a minimum of five years as a Senior Laboratory Technician. Minimum of two years working as a Department Supervisor.

Eligible for Registered Polysomnography Technologist (RPSGT) testing by Board of Registered Polysomnographic Technologist (BRPT); needs to obtain within one year of assuming this post.

Maintain current American Heart Association CPR certification.


JOB DESCRIPTION: LABORATORY PSG TECHNICIAN LEVEL I

SLEEP DISORDERS CENTER

PURPOSE:

To perform standard sleep related testing.

DUTIES:

Perform PSG, MWT and MSLT protocols.

Maintain a clean and safe laboratory and remove dirty linen.

Assures that all capital equipment is cleaned and cared for daily to ensure proper working conditions and a safe environment.

Participate in in-service programs and educational programs.

Interact with physicians, nurses, patients and other personnel in a courteous and professional manner.

May be asked to perform other duties in accordance with experience and qualifications.

SUPERVISION RECEIVED:

Work under the direct supervision of the Laboratory Coordinator.

SUPERVISION GIVEN:

None

QUALIFICATIONS:

High school graduate. Two years of college level work in the biological sciences or two years of work experience in a Clinical/Healthcare setting preferred.

Maintain current American Heart Association CPR certification.


JOB DESCRIPTION: Administrative Assistant

SLEEP DISORDERS CENTER

PURPOSE:

To perform clerical support duties including but not limited to medical transcription, telephone contact with patients to schedule appointments, and field concerns for staff and physicians.

DUTIES:

Receives and screens incoming telephone calls and routes them to appropriate individuals or records and relays the messages as needed. Schedules appointments and preregisters patients for various sleep studies, clinic visits and facilities visits including proper instruction to patient/family. Verification of patients insurance and obtain authorization for procedures.

Types, copies, faxes, and distributes material in order to meet the required turn around for ensuring proper medical attention to patients. Patient data entry into PC Windows customized database. Maintaining patient hard copy files.

Forward technical or unusual problems to the Clinic Coordinator.

Ordering office/clinic supplies.

Interact with physicians, nurses, patients and other personnel in a courteous and professional manner.

Participate in in-service programs and educational programs.

May be asked to perform other duties in accordance with experience and qualifications.

Good interpersonal communication skills and an understanding and sensitivity for patient confidentiality.

Assure that the clinic office is kept clean, tidy, and free of trash.

JOB DESCRIPTION: CERTIFIED REGISTERED NURSE PRACTITIONER

SLEEP DISORDERS CENTER

General Description:

Position will report directly to the Division of Pulmonary and Critical Care Medicine’s Sleep Disorders Center Medical Director. Position will work closely with the Director in primarily the outpatient clinic setting. Responsibilities include those of a Nurse Practitioner working in an outpatient setting such as new patient assessment, return patient follow-up and ordering of testing, equipment and medications as indicated at the time of clinic visits and between patient visits to the clinic. Clinical skills include ability to complete detailed history and physical examination for both new and return patients and dictate clinic notes from their visit. Training will be provided specific to the needs of the Sleep Medicine population to learn and implement diagnosis and treatment guidelines for this patient population. Self-directed study and learning may also be necessary. Work closely with other division staff to coordinate other aspects of patient care such as scheduling of clinic appointments, scheduling of necessary testing, trouble shooting CPAP problems and answering patient questions.

When applicable, the successful candidate will work directly with the Sleep Disorders Center Medical Director in the identification of appropriate subjects for enrollment into research studies, screening the subjects for eligibility, obtaining informed consent, and in arranging for the appropriate entry studies and follow-up visits.

Qualifications:

BS degree in nursing required. Completion of accredited Nursing School and Nurse Practitioner program. Maintains certification by attending appropriate continuing education classes or enrolling in formal credit classes. Current RN and NP licensure in the State of Maryland. Three years prior work experience as a Nurse Practitioner in a relevant Pulmonary specialty or specifically Sleep Medicine environment. Advanced clinical skills in specialty area or equivalent. Research knowledge including interpretation, utilization, and conduct of studies. General computer knowledge and capability to use computer. Ability to incorporate into practice theoretical concepts and advanced clinical skills with patients, families, and interdisciplinary health care team members. Analytical ability necessary to diagnose patient illnesses and to develop and implement plans of action for long and short term management of acute and chronic illnesses and to develop and implement plans of action for long term management of acute and chronic health care. Advanced interpersonal skills. Ability to communicate effectively, verbally and in writing, in order to provide information to patients and health care personnel. Ability to function effectively in a fluid, dynamic, and rapidly changing environment. Knowledge of JHMI clinical software such as Electronic Patient Record and EPIC scheduling system preferred. Knowledge of other JHMI clinical resources such as online databases and communications software also preferred.
