INTRODUCING PHYSIOTHERAPY

Fysio op Maat Tilburg-West is situated at the Conservatoriumlaan in Tilburg, near Tilburg University, in the same building as the (family) doctors Witkamp, Wijffels and Mees. In addition to general physiotherapy we offer a wide range of treatments: manual therapy, edema therapy and geriatrics physiotherapy. The practice also offers shockwave therapy. This enables us to adequately treat nearly all conditions of the musculoskeletal system.

All our physiotherapists are registered with the KNGF and in the BIG-register.

In our practice we can also offer acupuncture. Our acupuncturist is registered with the Nederlandse Vereniging van Acupuncturisten. (The Dutch Society of Acupuncturists).

Our staff speaks Dutch as well as English fluently.

How to make an appointment

You can make an appointment with a physiotherapist whenever you have questions or problems concerning human function or movement. It is possible to make an appointment with or without being referred by a (family) doctor /and or specialist.

You can book an appointment at the practice itself or by e-mail. We need your name, date of birth, address and telephone number. When making the appointment it is important to briefly explain the complaint/symptoms as clearly as possibly . If you wish to be seen by a specific physiotherapist (e.g. male/female), please inform us so we can meet your needs. We aim at a first appointment within 3 working days.

At your first appointment we are legally required to ask you for your ID (identity card, passport or driving licence) and have to register the number in the patient file (EPD). You also have to show us your health insurance card as proof of insurance so we can file that information .

First appointment without a doctor's referral letter

As of 2006 the physiotherapist is directly accessible for a screening (brief examination), therefore a referral by a doctor is no longer required.

The screening takes about 10 minutes. Afterwards will be assessed whether your complaint needs further examination and/or can be treated by a physiotherapist. Or whether a visit to a doctor is a better option. If the screening shows that physiotherapy cannot help your complaint, you will be referred to a doctor in most cases.

If the screening shows that physiotherapy is the right method of treatment, there will be a follow up screening.

This will be a further physical examination and the physiotherapist will draw up a plan of treatment together with the patient. In this plan for treatment the aim of the treatment (what do you want, what can be done and in what time), the form of therapy and the number of sessions will be noted. Any other methods of treatment will be discussed as well. As a rule the second appointment will be the start of the actual treatment of your complaint. Your family doctor will receive a short report of the screening. If you object to this, please inform us.

The first appointment with a doctor's referral letter

During the first appointment your complaints will be discussed and are followed by a physical examination. Afterwards the physiotherapist will draw up a plan for treatment with the you. In this plan for treatment the aim of the treatment (what do you want, what can be done and in what time), form of therapy and the number of sessions will be noted. Any other methods of treatment will be discussed as well. The examination will take about 30 minutes. As a rule the treatment will start at the second appointment.

Treatment

-  With your physiotherapist you have drawn up a plan for treatment at your first appointment. The treatment is specifically aimed at your complaints/condition.

-  It is important that you tell us what you expect every time you start a new treatment. This way we can check if the treatment meets your expectations.

-  By carefully evaluating the results of the treatment and by adapting the therapy to changes in your condition, we may not give you the same treatment at every appointment. This also applies to the advice and/or guidelines on lifestyle and exercises we give you.

-  If at a certain stage of the therapy your complaints are such, your physiotherapist can decide to consult a colleague or have the treatment continued by a colleague. However, only after we have discussed it with you. This method of work enables us to use all the expertise present in our practise.

-  During treatment a lot of attention and care will be paid to giving good information and advice.

-  The number of sessions may vary per client and condition and depends on the nature of the complaints and the recovery process. Your personal motivation and effort are also important.

-  When required we will contact your (referring) doctor during therapy, only after consulting you.

-  To make sure your treatment will not be interrupted, colleagues of the practice will step in for each other in cases of holidays, study or illness. When due to circumstances a physiotherapist is not available for a longer period of time, a physiotherapist from outside our practice might be employed to take over clients.

-  Our practice offers training places for student physiotherapists. It is possible that a student is present during your sessions or takes over (part of) your treatment under the strict supervision of the supervisor(s). As a rule, only after your consent.

-  As a client we expect you to take good care of your personal hygiene and that you take into account the rules for hygienic body care.

-  Your treatment is always ended with a report to your (family) doctor. This report is always discussed with you before we send it. And if you want one, you can always get a copy.

Payment

It is important to carefully read the policy conditions of your health care insurance to make sure you are covered and how many sessions are reimbursed.

We have contracts with nearly all health care insurers. It could occur that not all or no sessions are reimbursed because you are not or not adequately insured, or you have already had a number of sessions with another physiotherapist. You will then receive a bill from us at the end of the treatment. The rates and conditions of payment are available on our website.

It is always necessary for foreign students to check with their health insurance company how physiotherapy is covered in their policy before they start the treatment. Foreign students will always have to pay in cash at the end of their treatment and get a bill they can send on their health insurer.

This does not supply to students with a Dutch health insurance which covers fysiotherapy.

Cancellation of your appointment

When an appointment, for whatever reason, has to be cancelled, you will have to let us know 24 hours before to the appointment. If not, we will have to charge you for the appointment and you will receive a bill at the end of the month. You can cancel your appointment by phone, leave a message on the answering machine or send an e-mail.

Complaints and suggestions

FysioMaatwerk want people to be satisfied with their service. That is the reason why we continually work on the quality of our service. If you have a complaint or even a suggestion, please let us know. We will listen to your complaint and we will try and find a good solution. In the waiting room an extra leaflet (a complaint about your physiotherapist) will be available. A complaint will be dealt with according to the guidelines of the Royal Dutch Society for Physical Therapy(KNGF).

Privacy policy Fysio op Maat

The physiotherapists of Fysio op Maat Tilburg-West register all your medical and administrative data in order to be able to give the best treatment possible. The practice is bound by the Privacy Act. This act contains all rules and regulations concerning the protection of personal data (the Personal Data Protection Act (WBP), such as the purpose for registering these data, the nature of the data, administration of the data, the people who have access to the data and the right of the patient to view his file. All these issues are described in this Personal Data Protection Act.

Naturally, your physiotherapist will deal with these data with the utmost care. For that reason some extra rules and regulations have been agreed on in our privacy statement.

Apart from your treating physiotherapist a limited number of other professionals can access your data. E.g. a substitute physiotherapist, other physiotherapists in our practice, trainees/students and our secretariat. All these people our bound by their obligation of professional secrecy.

Only those data concerning your treatment will be enclosed in your medical file.

You have the right to view the data in your file. If you believe the information in your file is not correct, you can ask your treating physiotherapist to change it.

Personal data will not be disclosed to third parties, not even family. Data will only be disclosed after the client's explicit permission. Even after death the data will not be made public.

After finishing your treatment, the data will be filed in such a way that only the group of people mentioned above will be able to consult the data.

If you believe that our practice has not dealt with your data accordingly, you can file a complaint, by using our complaints procedure.
