Central New Hampshire VNA & Hospice, an award winning leader in the NH non-profit community, seeks an innovative Director of our Hospice Program to lead a highly professional interdisciplinary team. The agency has a strong commitment to ethical practice, and it strives to work collaboratively with other community agencies to improve the quality of life for residents and visitors. Understanding that we have an important role in revising the current models of care, CNHVNAH is large enough to launch innovative ideas and small enough to adjust rapidly to changing practice dynamics.
The Director will lead a team of 15 professionals and a corps of volunteers in two offices to provide high quality, home-based hospice & palliative care services throughout a mostly rural service area. The position will report to the agency Chief Executive Officer and will advise senior management on issues of strategic importance. This is a full-time, salaried & benefits eligible position.

Position Summary:
This position will assume overall responsibility for the coordination, implementation, and supervision of the hospice program – with responsibility for both clinical and financial performance. We seek an innovative leader, passionate about hospice care, well versed in federal regulations, nimble with the electronic healthcare record and interested in maintaining and building professional relationships with the community and its referral sources. The position will also work in partnership with the agency’s home health and pediatric programs to build a collaborative practice model that can be responsive to the community’s changing needs.

Qualifications:
Graduation from an accredited school of nursing, Bachelor’s degree required. Masters degree preferred.
Valid, unencumbered license to practice nursing in the state of New Hampshire
Valid license to operate a motor vehicle with no moving violations within the previous 2 years
Minimum of 5 years experience in hospice
Minimum of 2 years in a managerial role in healthcare, ideally in a hospice environment.
