SUBJECT: H1N1 Flu Vaccine
DATE: October 8, 2009
The Public Health Department has released the consent form and flu vaccine information sheets for the H1N1 vaccine. Please read the attached information sheets and if you decide that you would like to receive the H1N1 vaccine, please complete and return to your school nurse or Nurse Coordinator by Friday, October 16, 2009. The dates for the vaccine have not been determined but once a date has been established, you will be notified.
Please remember that this vaccine is not required and is being administered as a public service through the local Health Department.
It is recommended that the following groups receive the H1N1 vaccine:
· Pregnant women
· People who live with or care for infants younger than 6 months of age
· Health care and emergency medical personnel
· Anyone from 6 months to 24 years of age
· Anyone from 25 through 64 years of age who have certain chronic medical conditions or a weakened immune system
· The vaccine will be available to all employees (regardless of age) if it is desired.
If you should have questions or concerns, please contact your child’s school nurse or the nurse coordinator at 634-4753.

October 14, 2009 / Instant Alert to remind parents to return H1N1 vaccine form.
October 16, 2009 / Vaccine consent form due to School Nurse
October 28, 2009 / Projected date of first injections at school (Dates to be determined and parents will be notified)
