Directions to the Office and First Visit Instructions
Welcome. Thank you for choosing Westlake Orthopedics for your musculoskeletal care. This introductory letter should help your first visit to go much smoother. You should download all of the patient “office forms and policies” from the website. By accurately filling them out before your visit, you will alleviate much of the stress usually associated with the first visit to a physician’s office.
Westlake Orthopedics’ goal is to provide you the best quality medical care in a warm and caring environment. If you have any questions regarding office policies and procedures, please feel free to ask any of the office staff

When you arrive for your first visit please have all of your paper work filled out which will include the Patient and HIPAA policies and disclosures, Arbitration Agreement and the Insurance Authorization / demographics sheet. The HIPAA sheet is for your personal records.
It is important that you bring the following:
●Your active Insurance Card
●Your driver’s license
●Any referral papers from your physician
●A complete list of all your current medications and your medical problems
●A method to pay your co-payment (credit card or check is preferred). Your co-payment is due when services are rendered. It is not billed!
Please note that it is your responsibility to bring any pertinent medical information regarding your condition to the first visit. This information will support the accurate and rapid diagnosis of your condition as well as the initiation of the correct treatment plan.
Do not assume that someone else, i.e. the hospital or your doctor, sent these items to our office. Almost 100% of the time that assumption is incorrect. If you want it done, you will have to hand carry the following items to your appointment.
●Any and all x-ray, MRI and CT scan films as well as the accompanying reports pertaining to your condition.
●Any operative reports or Emergency room reports pertaining to your condition.

Get off of Thousand Oaks Blvd./Hillcrest Dr.
Make a Right on Thousand Oaks Blvd.
Make a Left on Clay Ct.
Make a Left on Jensen Ct.

Get off Rancho Rd. Get off Rancho Rd.
Make a Left on Rancho Rd. Make a Right on Rancho Rd.
Make a Left on Thousand Oaks Blvd. Make a Left on Thousand Oaks

Make a Left on Jensen Ct. Make a Left on Jensen Ct.
Follow the road to the end of Jensen Court and make a left into the parking lot.
Helpful Hints: When you are coming from the 101, you will pass under the 23 freeway on Thousand Oaks Blvd. Clay court is the 2nd street on the left. Landmark on the Thousand Oaks Blvd., Gino’s Tratoria. You will be turning Left on Clay court. Gino’s is on the corner of Clay and Thousand Oaks Blvd.
The medical office building is located at the corner of Pierce and Jensen Court. Most map services and GPS systems will leave you at the corner of Jensen and Clay courts. You will have to follow Jensen court around the curve to find the building. Follow Jensen Court to the end and make a left into the parking lot.
If you want to use Mapquest or your GPS, input the Address 896 Pierce Court in your navigation system or Mapquest. This will bring you to the PierceCourt parking entrance of the Medical Building. The building has a green metal roof and green awnings. It is easily visible from the 101 freeway and the 23 to 101 north transition road.
To enter the building, go to the front where the two large columns are located. The double glass doors are unlocked during business hours. The elevator is located to the left of the building directory. Our medical suite is located on the second floor, 110 Jensen Court, Suite 2A, Thousand Oaks, CA.
There is plenty of parking around the building and the lot is accessible from both Pierce and Jensen courts.
