First Nation/Aboriginal System Navigator – Full Time Contract Position

N’Mninoeyaa Aboriginal Health Access Centre invites applications for the position of a Registered Practical Nurse to work withthreeof our First Nation communities; Batchewana First Nation, Garden River First Nation, Thessalon First Nation and the urban Aboriginal population residing in Sault Ste. Marie.

As one of ten Aboriginal Health Access Centres in Ontario, N’Mninoeyaa Aboriginal Health Access Centre provides a combination of traditional healing, primary care, Health Promotion programs, Mental Health and Community Support Services to its member First Nation communities situated along the north shore of Lake Huron, between Sault Ste. Marie and Sudbury.

We are seeking a highly motivated, results-oriented individual to join our multi-disciplinary team of regulated health professionals. First Nation/Aboriginal System Navigator is to work collaboratively with the Hospital Discharge Teams within each of our partner hospital(s), to provide a full range of hospital discharge services and patient advocacy for seniors to ensure continued care upon discharge home. Travel to communities and hospital(s) will be required, travel costs are covered by the organization.

QUALIFICATIONS

Post-secondary Registered Practical Nurse College Diploma from a recognized college program.

Current registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse required.

Demonstrated experience in hospital setting is essential.

Ability to work independently with limited direction and supervision and cooperatively in a busy multi-disciplinary environment.

Excellent interpersonal, time management, communication and decision-making skills.

Proficient computer skills, EMR and Meditech experience an asset.

Demonstrated sensitivity to and knowledge of the Aboriginal cultural values and traditions. Knowledge and ability to work with people with diverse practices, cultures and beliefs.

A clear criminal reference check is a condition of employment.

Must have access to a reliable personal vehicle and a valid driver’s license.

Given the traditional practices of Aboriginal people, from time to time exposure to smoke from the burning of sacred medicines; tobacco, sweet grass, sage or cedar, may occur.

Salary: $42,000 to $46,800
