Embracing self-care – one year on event

Introduction

We want the health and care system to support people to have the knowledge, skills and confidence to take an active role in managing their own health and to work with communities and their assets. This includes self-management (what happens for the time spent without a care professional) and shared decision making (what happens for the time spent with a care professional).

Self-management

Self-care or patient self-management is what happens for the duration of time people spend without a clinician or other care professional. It is the majority of an individual’s time and the choices and decisions made during this time have a material impact upon the care outcomes for the individual.

Shared decision making

Shared decision making is the conversation that happens between a patient and their health professional to reach a healthcare choice together. This conversation needs patients and professionals to understand what is important to the other person when choosing a treatment.

The evidence base suggests that providing better support for individuals to help them manage their health effectively, whether in collaborationwith clinicians or individually, can have a material impact on health and well-being. It also allows health resources to be utilised more effectively for those circumstances where self-management is not appropriate. Innovation exchange

Eastern AHSN is hosting an innovation exchangeon 16 November 2017 which brings together citizens, the NHS, Local Authority and innovatorsto see what opportunities exist locally to accelerate improved self-management and shared decision making. The event will also explore new opportunities for partnership working as well as showcasing fantastic projects already happening in our region, some of which are the result of partnerships forged at our inaugural self-management event last November.

The event will take place during self-care week, an annual national awareness week that focusses on establishing support for self-care across communities, families and generations.

The exchange will feature innovators and existing projects in four zones as outlined below:

  • Mental Health;
  • Behaviour and coaching;
  • Exercise and wellness; and
  • Medicines management

Invitation

We would like to invite you to participate in this event and give you the opportunity to show case your innovation in one of the zones outlined above. Delegates will be asked to select innovators they are specifically interested in understanding and speaking with and will be booked into the zones for allotted times. This form of match making ensures that the conversations had on the day are meaningful and more likely to contribute to a future partnership arrangement than traditional market place events.
