Please give this form to your project mentor to read and complete. Form should be submitted to House Advisor. Please remember that mentors must not be a family member, must be 23 years of age, and must be an expert in the area of your project topic.

Information for all Community Mentors:
All students graduating from high schools in Iredell-Statesville Schools are completing a Graduation Project as the culminating assignment. The Graduation Project can take many forms—a work of art, a science experiment, a community service project—but it is a long-term project based in an area of interest for the student. Each student is required to have a mentor to offer consistent guidance and feedback and to monitor the student’s progress along the project timeline. As a mentor, you will:
- Be given a guide to become familiar with requirements for the Graduation Project and your responsibilities as mentor
- Become familiar with the deadlines and assessment criteria for the Graduation Project
- Allocate time to meet with your student on a regular basis (recommended five mentor/student meetings) to monitor progress, give feedback, and offer suggestions for the research paper, product, portfolio, and oral presentation
- Get in contact with the student’s academic advisor if your student is not fulfilling the requirements for the project
- Provide accurate and honest verification of the student’s work
- Complete a final evaluation of your student(s)’ work using provided rubric
- Sign and return all required forms
