Ted joins to refresh us on the cancer reporting problem of tying multiple reports from disparate labs together without having adequate linking information in the reports = this is related to derived specimen handling, which we also plan to discuss on the LabMCoP call on 9/7/2017 12 -1 PM EDT – Ted will try to join that call and bring some of the NAACR folks with him.
Example when you send out an isolate and need to know the original source site, or a slide of an AP specimen, where you need to know the block, the organ, body site etc
Ted’s problem is linking report fragments coming into the cancer registries from different labs:
Biopsy in hospital – sample to nearby local lab and they do a send out to lab in TN for biomarker study, and a send out or special stain to another local lab – each lab sends all reports as v2.5.1 NAACR Vol 5 format – using their own IDs
Biomarker report arrives before primary report from the hospital – linked by patient IDs only, but had different histologies from different body parts that were sent to different labs – so tried to link by collection date (but often the same date / sometimes not and not a good proxy) – need more information about the original sample (and the steps inbetween)
Folks have agreed to use SPM-3 to send the parent-IDs and SPM-30 (Accession number – CX datatype and repeats) or SPM-31 (other Specimen IDs – CX datatype and repeats)
BlockID may have been assigned and the lab reports on that
Example multiple–organ resection with multiple blocks from each organ created multiple nested structures = Page 259-260 NAACR guide Vol 5 use that! – or more complicated on page 271 – multiple sides and multiple primaries (de-identified actual case from Canada)
Regulatory issue is that for each sample there has to be a separate report
Does the reference lab have to report directly to the patient’s location state cancer registry – the specimen collector is on the hook for the reporting, but CA cancer registry requested the HL7messages directly from the reference lab for biomarkers, so since both are now sending the same reports.
We will also look for a date at HL7 WGM in San Diego in September that we can schedule an official lunch meeting joint Vocab / OO for this topic
