As per Diocesan Policy, a Background Check and/or a MVR were submitted on behalf of the parish or school on ______, 2016. Based on information received in the reports, it was determined that the employee has been cleared, but with driving limitations.

Catholic Mutual Group (CMG) requires that anyone driving on behalf of the parish or school is to submit information regarding their driver’s license and insurance. CMG will notify the diocese if a driver is not an approved driver, if underwriting receives information regarding citations or convictions in the past three years. A list of possible reasons for the exclusion is attached. The diocese, parish or school does not receive the reason of the denial. To request information or to challenge the report can occur within 60 days of the receipt of the exclusion letter provided. The employee must be notified that he or she is not to under any circumstances drive on behalf of the parish or school.

A denial also can occur by the Diocesan Child, Youth and Protection dept. if something is flagged in the background check that possibly could affect driving. This could occur for serious offenses greater than three years.

This notice is to verify that the employee has been informed of the driving limitation imposed on them. It is to be made clear that this is not a permanent restriction. The employee can resubmit a Drivers Information Sheet once they feel their report should be free of any exclusion. The employee understands that until the driving restrictions are lifted, they will not under any circumstances to drive on behalf of the parish or school. The employee understands that they are not to drive their own vehicle to conduct church or school business.
