Expectation: The rehab staff member will demonstrate correct & consistent patient instruction & staff use of selected outcome survey tools and will promptly & accurately score and document survey results

1. / Discuss the importance of including subjective surveys along with objective measurements when collecting patient outcome data
2 / Identify apaper & pencil survey to be used for the 2 psychosocial outcome measurement categories:
  • For Quality of Life (QOL):
  • CR = Dartmouth Coop survey
  • PR = COPD Assessment Test (CAT)
  • For depression status:
Both programs = Patient Health Questionnaire m(PHQ-9)
3 / Administer the QOL & Depression surveys onsite during the first week of rehab
4 / Provide clear explanation & consistent instruction to each patient taking the surveys
5 / Allow the patient to sit quietly to complete the surveys with as little assistance as possible:
  • Personal interpretation & spontaneous answers provide the most accurate data

6 / Score completed surveys promptly & accurately; enter those results directly on the Patient Outcome Data sheet
7 / For PHQ-9, use scoring grid to determine level of depression as:
  • 0-4 = normal
  • 5-9 = mild depression
  • 10-14 = moderate depression
  • 15-19 = severe depression
  • 20+ = very severe depression

8 / Enter psychosocial problems identified from surveys onto patient’s ITP for follow-up interventions during rehab
9 / Flag the need to repeat PHQ-9 every 30 days for any abnormal score (greater than 5) seen initially
10 / Near the end of rehab, repeat the 2 psychosocial surveys done at program entry
11 / Score completed surveys promptly & accurately; enter those results directly on the Patient Outcome Data sheet
12 / Review results with patient as part of outcome report during discharge interview; explain changes from pre to post rehab
