Meeting Minutes

Members Present: Leonard Andrews, Daniel Cluff Jr., Heriberto Crespo, Raphael Medina, Susan Jenness Phillips, Anna Poplasky, Gabriella Priest, Tim Perdue, and Jean Wright

Members Absent: Dick Guy, Leslie Nelson, Adrienne Methot, and Steve Levitt

Before Check-Ins:

·  Effective immediately, Roberto “Tito” Rodriquez has decided to step down from his position on the Quality Council. He is not able to devote enough time for it and knows that Len Andrews will represent the Youth Services department very well.

  • Check-Ins:

Heriberto:

·  CPR’s – The next meeting is scheduled for June 28, 2013. The CRP calendar was sent out for the next meeting date. Paul provided feedback on the CPR calendar; Heriberto will re-send in January 2014, as reminder to all, incorporating Paul’s feedback. The CPR Calendar will be posted on the SQA Web-site.

·  Currently working on SQA Monitoring.

·  Revamping Client File system at Sargent House.

·  Revising Youth Satisfaction Surveys.

·  Leonard and Tito’s proposal is due Friday, June 14, 2013.

o  Feedback for the DCF proposal due by 5:00 p.m. today.

·  STS is waiting for the DYS awarded notification due this week.

Raphael:

·  Tech Services Department updates:

o  Kenne Ningahi is transitioning into the Network Administrator position.

o  Tech Services has been utilizing a part-time intern (Mauricio Claret) until we start the hiring process for a Tech Support Specialist after July. Mauricio has prior experience here at CRJ.

o  We have contracted Wide Area Network and Virtual Private Network services from our friends at HMEA, Horace Mann Educational Associates.

o  The CRJ VPN (Virtual Private Network) data transfer process will be implemented in states, beginning July 2013 – and to be completed by December 2013. Including:

o  355 Boylston Street, integrating all users.

o  500 Harrison Avenue; and

o  All other programs locations and users throughout CRJ.

o  We are helping CRJ staff with the Encryption Policy implementation as needed.

Daniel:

·  Currently there are 17 shared living clients.

·  CS is opening a new day program in January. Members of this committee for the new program will be: Janet, Michelle, Terry, Yemi, Kelly An, Angelo, and Heather.

·  The day program has been implemented for sex offenders.

·  Opening in the Shirley office hopefully.

Tim:

o  The CS audits have been going well so far; most programs have been scoring very well. About ten audits have been completed so far.

o  Working on the PBO’s for DDS for Mass. The summaries will cover July 2012 – June 2013. They are being finalized and will be sent out the last week of June.

o  Going to CT first to complete the CT audit called QSR’s (Quality Service Review). Will be staying there for multiple days and working with the Simsbury and Windsor CT programs.

o  The CT audits are currently scheduled for once every six months; however this may change over time.

Jean:

o  MMR’s are mostly up-to-date with very few exceptions.

o  Youth Satisfaction Surveys have been tallied for STS in regards to Fall and Winter 2012, as well as Spring 2013.

o  CRJ Classifieds are in the final stages and we are anticipating a formal announcement regarding the classifieds soon.

Wally:

o  Rescheduled the QC-CQI make-up meeting for June 21, 2013 for members who were not present for Parts I and II. The meeting will be held from 10:00 a.m.to 4:00 p.m., 500 Harrison Avenue.

o  SMT Training will be held on June 26, 2013, from 11:30 to 3:30 p.m., at 500 Harrison Avenue in the large conference room.

Gabriella:

·  Currently interviewing for a Senior Associate position to do Pew juvenile work.

·  Boston Police Department contracted with CJI to find where people are transitioning to upon return from incarceration (as a result of the drug lab scandal) and what services are available to them; provided BPD with information, part of which will be posted on the CJI website soon.

·  CS – Collaborative process evaluation starting in July; CJI will go to the CS Manager’s Meeting in July to announce this to CS and answer questions.

·  Eight CJI staff volunteered at Sargent & McGrath House in May, in collaboration with Liz Curtin (SJS), and cleaned their yards. Hope to continue volunteer work in the future and perhaps open it up to 355 and 500 offices, if space allows.

Anna:

·  Registered for Boston Globe contest for employees. CRJ employees will receive a survey to complete.

·  CRJ ranked amongst the “Top 100 Employers to Work For.” Will keep the council updated.

·  Shaw Foundation – Submitted to ______plan to apply to Fallon Community Health (Wellness). Working on a proposal by the end of June.

·  Advancement Surveys to go out and the deadline is July 8th. Client profiles are on the CRJ Web-site.

·  CRJ Web-site – SJS and CS to work on the collaborative.

·  Mary Hawkes tribute will be up and running soon. Will be posting photos and memories regarding Mary Hawkes on the CRJ Web-site.

Susan:

o  Revisions in surveys – due for a revamping involved in the Performance Management. Heriberto is the first employee for the new system.

o  Jean will be starting full-time next month.

o  Tim has a new person to work with.

o  Met with Rick regarding strategies for Administration, Human Resources, and Fiscal.

o  IT and SQA are going to develop more ways to collect data. Determining what is the important data. Human Resources will collect the most amounts of data. Working on ways to measure success.

o  Strategic Planning is part of SQA. Will do its part on the analysis. The next Strategic Meeting in scheduled for July1st. The Executive Team and ______are planning better. Strategic Planning on located on the CRJ web-site; and is part of the CQI program.

o  Mary Hawkes brother passed away recently.

Quality Improvement Spotlight

The following commentaries were shared by QC Members regarding examples of Quality Improvement that were individually observed throughout CRJ:

Gabriella: EPIX Training - CJI and Watson Training

“People could not wait to get back to their programs to implement what they learned.”

Tim: Brookfield Residence

“Recent audit at the Brookfield – The program was in rough shape and has majorly improved. Overall, outstanding quality improvement.”

Raphael: Administrative Office and Brooke House Training

“The new intern has a new freshness and disposition and is a great resource with good skills.

“Blown away regarding their quality at Brooke House.

Heriberto: Administrative Office

“Medina helping the new intern on “how to use the copier”. She is very patient and provides step-by-step instructions. Janet worked with SQA and her staff to update MMR data.

Anna: Corporate Office

“Working with Bill to get the newsletter completed. Donations database (4 thousand to have good reports.)

Daniel: Community Strategies

“Quality is the Shared Living Clinicians – They had staff trained the next day.”

Jean: Administrative Office

“Quality Improvement is exemplified everyday within the Standards and Quality Assurance Department. Department supervisor and staff “go out of their way” to assist within all areas of SQA and CRJ as a whole.

Susan: Coolidge and McGrath House

“New program directors Joy Wingard and Dele Faly, as well as Dick Guy going over SQA expectations, MMs and accreditation. Met with Joy and went over MMRs. The next day received ACA news from Joy.”

Fishbone:

o  E-Academy not enough people using it.

o  Not being able to have on training unit.

o  How to get documented trainings from Maria?

o  Choose 1 area to work on immediately.

o  Create a database.

o  Perhaps invite Maria to the next QC meeting for training (current process).

o  Create a flow chart of before & after.

o  Everyone do a flowchart for the next meeting.

o  Raphael and Sergio to work on creating a database process.

o  Heriberto and Bill adding Watson to the database.

o  Handwritten flowcharts as a back-up.

o  Staff development and document training. Come up with a HR list.

ACTION PLANS:

o  Talk to Adrienne about a HR recommendation/add.

o  Discuss CQI plan at next meeting.
