COLWALL SURGERY PATIENT LIAISON GROUP MEETING

Minutes of Meeting held Wednesday 2nd October 2013, 6.30 – 7.30

PRESENT

Mrs Lisa Hinchliffe (LH) (Chair), Mrs H Philpott (HP), Mr M Allen (MA), Mr R Knight (RK), Dr Sarah Newey (SN), Mrs F Nikitik (FN) & Mrs Lynda Gittings (LG) Admin.

1. APOLOGIES

2. DECLARATION OF INTEREST

There were no Declarations of Interest.

3. MINUTES OF LAST MEETING (17/07/2013)

The Minutes of the last meeting were accepted as a true record.

4. MATTERS ARISING

ACTION (FN) Due to Surgery Laminator breaking down, Surgery Appointment Times will be Laminated by next Meeting and displayed in Lobby. Once Flu Clinic completed on 12/10/13, space for Surgery Appointment times/other information will be available on Prescriptions.

4.1 Patient Survey –MA suggested that the surgery make use of the local publication ‘Colwall Clock’ more for communicating information about changes to the surgery (e.g. appointments, etc.). All agreed this was a good idea and articles need to be submitted by the 10th of each month. Information already on Surgery Web Site and News Letter.

PLG had reviewed the questionnaire and agreed the questions were good. HP commented that the response rate needed improving following the low response last year and all agreed. FN said the next annual survey was due to be completed within the next 3 months.

4.2 Choose & Book – Comprehensive description given by Sarah Caldicott last meeting and notes of meeting circulated to group. MA concerned that the District Nurses were not based in Colwall and FN confirmed District Nurse’s based at LedburyHospital.A number of surgeries in the locality refer patients to the DN’, who then allocate their workload accordingly.

5. FLU CLINIC – 12TH OCTOBER 2013

FN asked for PLG’s help with Flu Clinic, as it is also a Fund Raiser for Macmillan Nurses & Colwall Care Fund. Volunteers requested for making and selling of Tea/Coffee’s and Cakes. Posters in local shops & Surgery lobby asking for Donations of Raffle prize’s and Cakes for the day. All helpers to have name badges and be at Colwall Village Hall by 9.30am. (FN) Representatives from Age Concern, Red Cross & Herefordshire Carer’s also coming to event. Julie, Surgery HCA taking Blood Pressures. Usual Team of Doctor’s & Practice Nurses dispensing Flu jabs. Mop up Clinic for patients unable to attend the 12thNovember approximately 4 weeks after.

6. NHS UPDATE

MA asked if recent Government proposals of all Surgeries opening 8 -8, 7 days a week feasible? FNsaid that currently the scheme was being piloted in different areas nationally. However, the current practice income would make it very unrealistic to implement within Colwall. The group felt that patients still were unsure about how to book appointments and what was available. It was agreed that the surgery would review how to promote the different types of appointments and how patientscan contact the surgery.

FN reminded the group that the Herefordshire CCG has quarterly public meetings and the group expressed an interest in attending them in the future.

MA raised the issue of patients from across the Border in Wales. FN confirmed that Welsh border Patients requiring referrals resolved. Patients can be referred to either Hereford or WelshTowns.

ACTION (FN) to inform PLG members of CCG forthcoming meetings.

West Midland Ambulance Service to handle the call service for NHS 111 until March 2015.

7. HCCG PATIENT MEMBERSHIP (LH)

LH distributed the leaflet about the Membership Scheme for the CCG and encouraged members of the PLG to join. This is a platform for Patients to have their say, gives the opportunity for feedback and be kept informed of future agenda’s of the local NHS.

8. RISK PROFILING.

FN the aim of Risk Profiling is to anticipate and identify patients at possible risk in future days and months. Surgery already has in place Multi-Disciplinary Team meetings with Doctors and other local professionals to discuss Patients most needing further care (e.g. District Nurses, Occupational Therapists, Physiotherapists and Macmillan Nurse).

HerefordCity already piloting “Virtual Wards” this to enable a cut down of Hospital admissions. It would give a 24 hour care by a specially trained staff (Community Matron) and they they would decide, with the patients GP, who is eligible (Chronic patients only not Acute). Nurse would possibly visit daily (depending on the needs of patients) and develop patient Care Plan with the GPs help. Currently, the pilot is in the eight city practices (Hereford). The Risk Profiling tool would identify patients who may need extra care and the GP and Community Matron would review this list. New patients can opt out if they don’t want their information sharing.

9. ANY OTHER BUSINESS.

ACTION (FN) to inform Lisa of future Herefordshire Carers Meetings.
