CO-OP STUDENT AGREEMENT
The Co-op Student agrees to: perform all assigned duties to the best of her/his ability; satisfactorily meet all requirements of the employer, her/his academic department; abide by the rules, regulations and policies of the employer and the College; pay all applicable College fees while on assignment; register for the appropriate Work Experience course billed at the regular rate; and abide by the Standards of Professional Conduct listed on the reverse side of this agreement. Should she/he unjustifiably fail to meet these requirements, she/he may be withdrawn from the co-op assignment, remain responsible for any financial obligations and forfeit any academic credit provided for her/his participation in the XXXX Dept.Cooperative Education Program.

CO-OP EMPLOYER AGREEMENT
The Co-op Employer agrees to: coordinate the student's assigned duties so that her/his work will be closely related to the student's academic degree program and/or career objectives; provide supervision of the student; evaluate the student's performance at the end of every work assignment utilizing forms provided by the XXXX Dept. and inform student as to status of next work term; provide for a faculty on-site visit each semester for the purpose of evaluating student's progress; and provide the student the same consideration of health, safety and working conditions accorded all other full-time employees.

The Co-op Program agrees to: maintain communication with the employer and the student in an effort to answer questions, resolve potential problems and otherwise endeavor to make the Co-op experience as productive and rewarding as possible for both the employer and the student.

COOPERATIVE EDUCATION
STANDARDS OF PROFESSIONAL CONDUCT
1.While on the Co-op Work Experience, the student is an employee of the company or agency and is under the supervision of that organization in the performance of her/his duties. Students must meet the employer’s standards of quality, loyalty, confidentiality and appearance and must seek to contribute to the improvement of the operation of the firm or organization. As employees, students are expected to practice the highest standards of professional ethics.
2.Each student is expected to work as scheduled and recorded on the work agreement by the employer. A student observes only company holidays, not College holidays or vacations unless other arrangements have been made with the employer.
3.Each student is required to register for their Co-op work experience at the beginning of each semester (fall, spring and summer).
4.In case of illness or other emergency which requires a student to be absent from work, the employer should be notified as quickly as possible by telephone. If an absence will cause the student to miss a week or more, the Faculty Co-op Adviser must also be notified.
5.It is not ethical for a participating Co-op student to request or receive unemployment compensation based upon periods of Co-op Work Experience. Such application would be in violation of State laws and would be a breach of the good-faith understanding between the student and the Co-op employer. Any student making an application for unemployment compensation will be subject to immediate suspension from the Co-op Program.
6.Upon the acceptance of a Co-op Work Experience, the student is required to remain on the assignment until released by the Faculty Co-op Adviser.
7.All Co-op students must maintain academic standards by meeting the required quality-point average to enter each succeeding year of their academic program. Students with deficiencies in course work or in scholastic average should inform their Faculty Adviser. The student’s continuation in the Co-op Program will be determined jointly by the department chairperson and the Faculty Adviser.
