This template can be also used for recording other forms of contact with the young person, e.g. a phone call

The PA should record the visit on ESCR within 2 working days stating clearly:

a.  Who was seen and where

The young person, parent/carers, partners, friends, other support network etc. Was the young person seen (at home, college, work, relatives etc.). If the young person was not seen at home, state reason.

b.  What the young person’s views are about how things are for them

Give details of quality of interaction and communication type to give picture of the young person and to ensure interaction is captured. What does the young person say about how they are? Where possible use young person’s own language. Capture their views and feelings especially around Pathway Planning and for contributing to the next review of the plan.

c.  Assessment of the young person’s welfare, health, social needs, and education, employment and training. Comment on:

·  specific areas identified within the young person’s Pathway Plan;

·  General and specific health needs;

·  Welfare needs – racial, cultural, linguistic needs;

·  social needs including leisure;

·  education, employment and training status and needs;

d.  Assessment of budgeting and financial needs;

Include details of receipt of benefits; bill payment, housing arrears

e.  Assessment of suitability of accommodation

Is the young person appropriately housed? If not, what are the plans to address this?

f.  Assessment of relationships and contact with others

Include details about who makes up the young person’s support network; what contact do they have with family, siblings and friends; what can be done to support contact if this is not taking place

g.  Comments made by any other person present at the visit

h.  Analysis of the visit, including concerns and strengths

Use evidence from observations and theory where appropriate to support analysis of the case. What areas of strengths have been identified

i.  Actions identified during the visit

Agreed actions, by whom, by when, and how is this to be measured. Any changes to the Pathway Plan.

j.  Date and Time of next Visit:

Confirm the next visit or other form of contact with the young person,

Write this on a visit card and give it to them.

Copy this recording of the visit into the ESCR activity and record as:
