The AZ HOSA State Spring Leadership Conference will be held at The Hotel Arizona in Tucson beginning Monday April 21st and ending Tuesday April 22nd at the close of the awards ceremony.

REGISTRATION
The registration for the conference is on line. On February 5, 2008 you will receive your log in and password. All other important information can be found on the AzHOSA web site. All participants (students, advisors, and chaperones) must be registered. Follow the “How to Complete the Registration Form” document to register your participants

1. / Complete On Line registration
The registration deadline is Friday, March 14, 2008 by midnight
2. The registration fee is $45.00 for ALL students. Advisors and Chaperones
are FREE.
3. You are required to pay a registration fee for every student on your original online registration. No deletions or refunds will be made after the conference registration deadline.
4. / Print a copy of your emailed registration and send it with a check (made payable to AZ HOSA) for your fees to:

5. / Print one copy of your registration for yourself.

CONFERENCE FEES ARE DUE 1 WEEK PRIOR TO CONFERENCE DATE. You are required to pay a registration fee for every student on your original online registration. No deletions or refunds will be made after the conference registration deadline.

ONSITE CHECK-IN
Onsite check-in will be from 8-9 AM on Monday, April 21st at the The Hotel Arizona Grand Ballroom Lobby. Advisors will be issued their conference registration printout, nametags, programs, maps, etc. Team substitutions may be made only during onsite check-in. During onsite check-in the following items will be collected:

·  PSA envelope with all forms and DVD

·  Researched Persuasive Speaking Essay

·  Student Eligibility Forms (***for competitions marked ***)

·  Health Education Notebooks

SLIDE SHOW
Send your pictures of chapter events to Jane Shovlin by March 20, 2008 in order to have them displayed during the slide show. The slide show will take place on Monday prior to the opening ceremony. The pictures will need to be emailed to

NAME TAGS
Conference name tags must be worn at all times during all HOSA activities.

VOTING DELEGATES
Each chapter may register two voting delegates for the conference. Voting delegates must check in and wear their ribbon and sit in the reserved section during the business session.

COMPETITIVE EVENTS
Be sure your students have carefully read the guidelines for their competition. Please prepare students to have a positive attitude. They should understand that the true benefit of competition is in the preparation, participation, and networking - and not only in winning.

COURTESY CORPS
Each advisor should designate one or two students to serve as courtesy corps. These students should be available during your assigned competition. Courtesy corps students will assist their advisor in running the advisor's assigned event.

ADVISOR RESPONSIBILITY
All advisors must participate in conference management by sharing in conference duties. Failure to complete designated duties may result in the advisor appearing before the Board of Directors, which may result in the disqualification of the advisor’s students.

CODES OF CONDUCT AND MEDICAL LIABILITY RELEASE FORM
Advisors are responsible for having their students complete the HOSA Code of Conduct form and Medical Liability Release form. A parent or guardian’s signature is required on both forms. Advisors should sign the Advisor’s Code of Ethics form. Please have all of these forms in your possession during the conference.

SPECIAL NEEDS STUDENT EVENTS
Student Eligibility Forms for students participating in First Aid/Rescue Breathing, Personal Skills, Speaking Skills and Interviewing Skills must be turned in during onsite registration when you pick up your packet. If accommodations need to be made for your special needs student, please indicate the exact accommodation during the online registration process. There is a section specifically for this. If your special needs student is wheelchair bound, please let us know.

JOB SEEKING SKILLS AND INTERVIEWING SKILLS
Cover letters and resumes must be emailed to Jane Shovlin at on or before the conference registration deadline of March 28, 2008. The cover letter MUST be in the body of the email and the resume sent as an attachment. The student should email the resume and cover letter. A confirmation email will be sent upon receipt of the competitors email.

AWARD PRESENTATION
All competitors attending the awards ceremony MUST be in professional dress. If a competitor is not dressed in appropriate professional attire, they will not be allowed on stage. If a student is unable to attend the awards ceremony to receive his/her medal, the advisor should designate someone to receive the medal for the student. No awards will be mailed to recipients.

CHAPERONES
The required ratio for adult supervision of students is 1 adult sponsor per 10 students for all HOSA activities.

LOST & FOUND
Any items (notebooks, purses, money, etc.) found should be taken to the Tabulation Room. Individuals having lost an item may claim said item from the Tabulation Room. Any items not claimed prior to the start of the closing ceremony will be discarded.

POSTER & NOTEBOOK PICK-UP
All Career Health Displays, Extemporaneous Health Posters, Outstanding HOSA Chapter scrapbooks, and Community Awareness notebooks must be picked up from the Tabulation Room at least one hour prior to the start of the closing ceremony. Any unclaimed posters or notebooks will be discarded.

CONFIRMATION OF REGISTRATION
You will send an email confirmation note as each registration is received. If you do not received confirmation within 2 days of emailing in your registration, please follow-up to make sure your registration was received.

2008-2009 OFFICER CANDIDATE TEST
The Officer Candidate test questions will be 60% HOSA and 40% Parliamentary Procedure. The questions will be taken from the following references:

• National HOSA Policy & Procedure Manual
