Nomination Information

Process:

With thecompleted nomination form, please provide the following:

  1. One 5 x 7 digital photograph of the nominee
  1. Brief written summary(max 250 words) highlighting the impact, creativity and dedication of the nominee’s contribution to arts and culture in Collingwood
  1. Support documents:
  • List ofaccomplishments, awards orachievements relevant to the nomination. If applicable, include a sample of the nominee’s work (copied, do not send originals)
  • Two written testimonials attesting to the value and impact of the nominee’s achievements
  • Additional materialthat supportsthe nomination

*Please note that support documents should not exceed 5 pages in total.

Eligibility:

  • Individual nominees must reside in Collingwood or have an ongoing association with Collingwood and have contributed significantly to arts and culture in the community
  • Individual nominees must be living at the time of selection
  • Organization or group nominees must be located in Collingwood
  • Nominations for the same individual or group may be received more than once

Nominations must demonstrate the impact, creativity and dedication of the nominee’s contribution to arts and culture in Collingwood through volunteerism, artistic leadership, administration or philanthropy.

Backgound:

The Awards Program was created in 2011 to recognize and celebrate contributions to arts and culture in our community.

Past recipients include Susan Connolly, Anke Lex, Barbara Sharp and Sharron Fleet in the Individual Category; Allison Gallagher, Anne Marie Marsden and Aidan Funston in the Youth Category (18 & under); and the Blue Mountain Foundation for the Arts and Georgian Bay Association for Creative Arts in the Group Category.
