PAM DEWEESE PERFORMING ARTS FUND GRANT APPLICATION

The mission of the Pam DeWeese Performing Arts Fund is to provide funding and support for the performing arts in the Avon Community School Corporation. The goal is to fund initiatives that support excellence, innovationand growth of the performing arts in Avon’s schools.Pam DeWeese Performing Arts Grants involve a one-step application process for requesting funds of no more than $1,000 for annual/school year programs and no more than $500 for programs with a limited scope. The number of Pam DeWeese Performing Arts Grants will be determined by the AEF Board of Directors and may change based on the amount of unused funds returned from previously awarded grants as well as the amount of additional donations made specifically to the Fund.

Pam DeWeese Performing Arts Grants utilize thePam DeWeese Performing Arts Grant application form – not the traditional AEF grant form and a budget worksheet that must accompany the Pam DeWeese Performing Arts Grant application form.

POLICIES

  • Applicants must be certified employees of the Avon Community School Corporation.
  • Individuals may submit no more than two (2) Pam DeWeese Performing Arts Grants applications per grant cycle.
  • Applications for Pam DeWeese Performing Arts Grants may include funding requests for educator stipends up to a maximum of $600 per school year.
  • Grants for technology will be considered; however, ACSC has provided the following caveat: ACSC cannot repair, replace, or incur the cost of services associated with certain technology devices (Ex: tablets) which are purchased with grant funds. Please feel free to consult the Technology Department with questions if clarification is needed.After the building principal signs off on the grant application, the teacher & building leadership are considered adequately informed about the caveat regarding equipment repair.
  • The Foundation does not fund incentive gifts or food for functions.
  • Grant applications will be accepted for the continuation of programs. Programs receiving Pam DeWeese Performing Arts Grant funding may be eligible for continued funding in subsequent grant cycles; however, funding in one grant cycle is not to be construed as a guarantee for future funding.
  • Grants are judged on a competitive basis and may not be funded in full.
  • Funding priorities will include (a) projects that address needs or program gaps resulting from reduced ACSC funding and/or loss of instructional time; (b) programs with broad-based implications and/or that exceed routine classroom activities or expectations; (c) buildings/programs/teachers without access to other external, i.e. non ACSC, sources of support.
  • Applicants for Pam DeWeese Performing Arts Grants are encouraged to include supplemental materials demonstrating need, program effectiveness, or desired outcomes. Supplemental materials may include, but are not limited to, photographs, videos, and/or testimonials.
  • Any funds provided for a designated program that are not used in full must be returned to the Avon Education Foundation, unless written approval has been granted by the Avon Education Foundation Board of Directors.
  • Grant recipients may be invited to provide a display of their grant in the future.
  • All items purchased with funds provided by Avon Education Foundation remain the sole property of the Avon Community School Corporation.
  • The grant recipient(s) must utilize their funds and complete and submit their grant summary form within one year from the date of approval.
  • While implementing and promoting the funded program, acknowledgment must be

given to the AEF. Please insert the following language and the Avon Education

Foundation logoin any materials related to the program:

“Funded by the Avon Education Foundation through the Pam DeWeese Performing

Arts Fund.”

APPLICATION CHECKLIST

  • Narrative letter should clearly:
  • Describe the project purpose andthe needthat your request is intended to address
  • Include goals, anticipated outcomes, and the plan for assessing outcomes
  • Identify the student population that will benefit from the grant as well as the broad-based implications that exceed routine activities
  • Provide examples of methods or procedures that will be followed in implementing the grant in your classroom or building.
  • List any state standards that are addressed and explain how your students will benefit.
  • Include a time frame as well as listing all items for which the grant monies will be used, including any additional information that may explain the specific budget worksheet (below).
  • Narrative Letter must be submitted on school letterhead.
  • Complete the application form & budget worksheet(scroll down to the next page) and secure an original signature from your administrator.
  • Paper clip together 7 copies of the entire package and submit to AEF office no later than the date listed in the Grant Cycle Schedule.
  • Send one electronic copy of the submission to Kerry Tuttle, Avon Education Foundation Executive Director
