SCHENGEN VISA application for Tourism/Visiting family or friends(max. 90 days for non Malaysian citizen)

Applicant to be present in person with this checklistand all the following required basic documents:

Schengen visa application form (inDutch / French / English) properly completed, signed and dated.
2 recent passport-sized photos (3,5cm x 4,5cm) in color with a white background.
Original Passport (+1 copy). It should be issued less than 10 years ago and valid for at least 6 months after the intended stay in the Schengen states.
Original Malaysian resident permit(+1 copy)Employmentpass / Work permit / Permanent Resident card / Student Pass / Dependant Pass ...Valid for at least 6 months after re-entry.
If you work or study: proof of approval for holidays (original).
Stating applicant’s current position, annual salary, starting date of employment and period of leave.
Your flight reservation (return ticket).
At the collection of your visa: your flight ticket (original + copy).
Proof of lodging, i.e. hotel reservations, rental of holiday home,
Or stay with a family member or friend:
* Original Invitation letter from the host in Belgiumstating name, address and period of stay + copy of the host’s resident permit (Belgian ID card or Resident permit for foreigners).
Proof of financial means in the country of residence: bank statements covering at least 3 months, credit cards, traveller cheques, tax income papers, real estate property…
In case a pledge from a guarantor (“Tenlasteneming” or “Prise en charge”) is signed in Belgium:
The guarantor of Belgian nationality must reside in Belgium. The guarantor of foreign nationality must be allowed to stay in Belgium for an unlimited time.
The document can be obtained from the guarantor’s municipality where it has to be certified. It is valid for 6 months after its issue.

Official proof of sufficient solvency of the guarantor: copy of the guarantor’s recent salary sheets, covering the last 3 months /guarantor’s most recent tax sheet/ other official proof of the guarantor’s solvency. Remark: a letter from the bank or deposit account statement will not do.

A photocopy of the identity card or the unlimited residence permit of the guarantor.

An original and authenticated Certificate of family composition on the name of the guarantor, issued by his/her municipality in Belgium.
Proof of travel/accident insurance to cover possible medical expenses, hospitalization costs and/or repatriation for medical reasons while travelling in Belgium or in other Schengen countries, with medical cover for at least € 30.000/RM140.000. You will have to show proof of travel/accident insurance when entering the Schengen area.
Visa fee(not refundable and subject to change)
Accompanying family members need to submit a separate and complete file.

Original Marriage certificate(+1 copy)
Copy Spouse’s passport
pledge of financial support + proof of sufficient financial means
FOR CHILD
Original Birth certificate(+1 copy)
Copy Passports of both parents
Original Letter from school for child above 6 years old
If the documents are not in French, Dutch, German or English, they have to be presented with a translation from a sworn translator.
