AMTA Fellow Nomination Form

AMTA Fellow is the highest level of membership in the AMTA organization and is named after Dr. Edmund S. Gillespie. The AMTA Edmund S. Gillespie Fellow membership level is awarded by the AMTA Board of Directors for significant contribution to the Theory & Practice of Antenna Measurement. Nominations can be made by members to the AMTA awards committee via the AMTA president or AMTA past president. These nominations will be evaluated by the AMTA Awards Committee and the AMTA Fellows Committee. Following approval by the AMTA Awards Committee, the nominations will be submitted to the AMTA Board of Directors for final approval. Nominations should have an extensive justification, including three letters of recommendation from AMTA Fellows.

Criteria

Please submit the following:

·  Describe contributions to AMTA in 2 of the following 3 areas:

1) Significant technical contributions through publications in antenna measurements.

{Example – List number of publications in AMTA, IEEE, books, etc.}

2) Excellence in education in the field of antenna measurements

{Example – List number of graduate students advised, short courses taught, papers written by students advised, etc.}

3) Dedication through active service for AMTA.

{Example – List AMTA service on BoD, host committees, volunteer committees, etc.}

·  Nominees must be recently involved in AMTA. The nominee should have held membership in AMTA for 3 of the proceeding 10 years.

·  Finally, the completed nomination form must be accompanied by (3) letters of recommendation from current AMTA Fellows validating the nominee’s candidacy. Instructions for recommendation letter are attached.

Nominations should be sent to the President or Past President of AMTA at and by July 15, 2014. A biosketch of the nominee can be submitted with this form for additional information; however, the criteria section of this form must be filled out and three nomination letters from AMTA Fellows must be submitted for consideration for AMTA Fellow.
