REGISTRATION REQUIREMENTS

Student must be accompanied by parent or guardian when enrolling.

A. Student’s birth certificate.

B. Verification of Residency: ONE DOCUMENT FROM EACH CATEGORY

BELOW IS REQUIRED(per Board Policy dated May 2008, residents in the Fremont

Unified School District must provide proof of residencyof the primary occupant or host).

1. Current Picture ID:
  • California Driver’s License
  • California State ID card
  • Government ID card
  • Valid passport or consulate issued picture ID
  • Credencial para votar Mexicana
  • ID from home country
/ 2. Current Original Document:
  • Valid California vehicle registration
  • W-2 Form
  • Pay check issued by employer with both employer’s and employees name and address imprinted on pay check
  • State agency mail, housing authority voucher, etc.

3. Current Original Property Related Document:
  • Property tax bill or Close of Escrow Statement
  • Rental or lease agreement name of landlord/ landowner’s name address and phone number must be printed on the documents
/ 4. Current Bills:
  • Utility bill/Cell phone bill
  • Car insurance bill
  • Official business mail (i.e. bank statement, health insurance document, credit card statement, etc.)

All FOUR documents provided MUST BE CURRENT and have the name of the primary resident printed on the documents, the current address, and the date issued.

  1. Immunizations verification: See attached requirement list.

For the 2011-2012 school year: ALL STUDENTS entering grades 7-12 are

required to provide proof of the PERTUSSIS (WHOOPING COUGH) Vaccine

booster (Tdap) given after the child's 7th birthday. Refer to the list for more information

D. Transcripts: showing high school credit. If enrolling from another school during the

school year submit withdrawal/checkout paper with transcript.

Important information to assist you in completing your enrollment packet

Caregiver Affidavit:

If student does not live with his/her parent;

contact Pupil Services for a Caregiver’s Statement. Please call for an

appointment at 510-659-2534 BEFORE ENROLLING at the school.

Declaration of Residency:

If the student and parent are residing with another family, and do not

have proof of residency; contact Pupil Services for a Declaration of

Residency. Please call for an appointment at 510- 659-2534 BEFORE

ENROLLING at the school.

Inter-District Agreement:

The student does not live in FremontUnifiedSchool District;

the parent must apply for an inter-district transfer request from the

school district in which they live. After paperwork is completed and returned

to the home school district office, an agreement between the two school

districts will be decided, and the parent/guardian will be notified of the decision.

Language Assessment:

The language assessment form is to be completed by the parent.

If any language, other then English, is written in questions 1-3, the parent

must contact Language Assessment at 510-659-2564 for testing after the student

registers.

Special Services:

If the student was in Special Education classes, please call Special Services at

510-659-2569 for assistance before the student registers.
