I. BACKGROUND OF ORGANIZATION

CareerSource Pinellas

WorkNet Pinellas, Inc. d.b.a. CareerSource Pinellas is a 501(c)(3) non-profit organization. The WorkNet Pinellas, Inc. Governing Board was appointed and designated by the Pinellas County Board of County Commissioners to act as the Pinellas County Workforce Development Board under provisions of the “Workforce Innovation Act of 2000”. WorkNet Pinellas, Inc. has requested and received certification as the Region 14 Workforce Development Board by Workforce Florida, Inc., the State of Florida Workforce Development Board. This public-private partnership supports and promotes economic growth through workforce development. WorkNet Pinellas, Inc. is one (Region 14) of twenty-four regional workforce boards in Florida.

The Board of WorkNet Pinellas, Inc., comprised of representatives from businesses in Pinellas County, local educational entities, labor organizations, community-based organizations, economic development agencies, one-stop partners, and from other individuals deemed appropriate, are appointed by the Pinellas County Board of County Commissioners and certified by the Governor of the State of Florida once every two years. Criteria for composition of the Board are set forth in Section 117, Chapter 2, Subtitle B, Title I of the Workforce Investment Act of 1998. The Board of WorkNet Pinellas, Inc. serves as the Administrative and Fiscal Agent for the region.

CareerSource Tampa Bay

Tampa Bay WorkForce Alliance, Inc., d.b.a. CareerSource Tampa Bay is a 501(c)(3) non-profit organization. The Tampa Bay WorkForce Alliance, Inc. Governing Board was appointed and designated by the Hillsborough County Board of County Commissioners to act as the Hillsborough County Workforce Development Board under provisions of the “Workforce Innovation Act of 2000”. Tampa Bay WorkForce Alliance, Inc. has requested and received certification as the Region 15 Workforce Development Board by Workforce Florida, Inc., the State of Florida Workforce Development Board. This public-private partnership supports and promotes economic growth through workforce development. Tampa Bay WorkForce Alliance, Inc. is one (Region 15) of twenty-four regional workforce boards in Florida.

The Board of Tampa Bay WorkForce Alliance, Inc. is comprised of representatives of businesses in Hillsborough County, local educational entities, labor organizations, community-based organizations, economic development agencies, one-stop partners, and other individuals deemed appropriate. All Board members are appointed by the Hillsborough County Board of County Commissioners and membership composition requirements are reviewed and certified by the Governor of the State of Florida once every two years. Criteria for composition of the Board are set forth in Section 117, Chapter 2, Subtitle B, Title I of the Federal Workforce Investment Act of 1998 and the State of Florida’s Workforce Innovation Act of 2000. The Board of Tampa Bay WorkForce Alliance, Inc. serves as the Administrative and Fiscal Agent for the region.

II. PRODUCT SPECIFICATIONS

This RFP solicits proposals from a vendor who can demonstrate successful and cost efficient distribution of prepaid debit cards. Prepaid debit cards will be purchased to support workforce programs at both CareerSource Pinellas and CareerSource Tampa Bay.

The anticipated usage of prepaid debit cards for a twelve month period is indicated in the table below.

The current projected funding for the requested service is a minimum of $700,000. In the event an award is made, the funding period will be through June 30, 2016 with an option to extend for up to three (3) additional years, based on performance and funding availability.

The standard denominations to include in the cost quote are $25, $50, $100, $175 and $200.Additionally, please identify if other denominations become needed will this be allowable and any costs associated.

The vendor will provide the details for the following:

1.  Process for ordering.

2.  Invoicing.

3.  Payment terms.

4.  Length of time for delivery of goods.

5.  Any delivery or surcharges.

6.  Any defined shelf life of gift cards or reduction of value.

7.  Return procedures.

8.  Price quotes and or quote reductions must be good for twelve (12) months.

9.  Identify cost increases or quote reductions applied, if any, after the first twelve (12) months.

10. Identify general discount information regarding purchases that are not included in the Cost Proposal.

11. Identify customer service features are included with your card service? For example, would customers and/or we have the ability to call a 1-800 number to obtain card balances or resolve card issues?

12. What use restrictions can be placed on cards, if any?

13. Describe experience and past performance record in providing prepaid cards as a vendor within the State of Florida from July 2012 to present. List the regions, types of services provided, length of service, performance data and contact name and contact information.

14. Is your organization registered as a Disadvantaged Business Enterprise (DBE) with the State of Florida?

15. What other denominations are available for the prepaid debit cards? Are the cards available in any denomination desired?

16. What, if any, requirements are necessary for CareerSource to fill in order to do business with your company for the purposes of purchasing prepaid debit cards.

III. PROPOSER QUALIFICATIONS AND REQUIREMENTS

Respondent must meet all the minimum qualifications outlined below.

1.  Have provided services similar in nature and complexity for at least two years.

2.  Have contracted to provide services similar in nature and complexity, with at least one organization within the past three years. Performance of similar services as an employee does not fulfill this requirement.

3.  Be licensed to conduct business in the State of Florida.

IV. RFQ TERMS & CONDITIONS

A. Response

Each Respondent is required to submit their proposal no later than 12:00 noon EDT on November 30, 2015 (due date and time). Only fully a responsive proposal will be considered. The delivery of the proposal is solely and strictly the responsibility of the Respondent. A proposal received after the due date and time will not be considered fully responsive and will be returned to the responding party unopened.

All conditions set forth in the Content section below must be followed to be considered fully responsive. Failure to respond to any required section of this RFQ may result in disqualification of the proposal.

1. Content:

Include the following information in your proposal in the order presented below.

·  Cover Page - Use Appendix A – Only forms that have been fully completed and signed will be considered fully responsive.

·  Appendix B – Cost Quote

·  Appendix C – Questionnaire

Do not include the full RFQ document in your proposal. Do not respond to any questions by referencing material presented elsewhere. The response provided immediately after the restatement of the requirement shall not be considered complete and stands on its own merits. A response of "will comply" or "see above" or similar statement shall be considered unresponsive.

2. Format:

Response must be typed, using a minimum of 12-point font size; single spaced and submitted on a single-sided 8 ½” X 11” letter size paper.

3. Quantity:

·  1 signed paper original. The original should be marked “Original”.

·  3 paper duplicates marked “Copy”

·  1 CD ROM disk or Flash drive copy of all documents included in the proposal.

4. Sealed package: No electronic mail (email) or facsimile (fax) copies will be accepted.

All documents become the property of CareerSource Pinellas and CareerSource Tampa Bay and will be a matter of public record subject to the provisions of Chapter 119, Florida Statutes. Each Respondent agrees that the preparation of all materials are at the Respondent’s sole cost and expense, and CareerSource Pinellas and CareerSource Tampa Bay shall not, under any circumstances, be responsible for any costs or expenses incurred by a Respondent.

B. Question and Answer Period

All requests for clarification of this RFQ should be emailed to by 5 p.m. EDT, November 20, 2015. Please reference RFQ No. 16-1097 in the subject line. A good-faith effort will be made to provide a written response to each question by 12 p.m. EDT, November 23, 2015. C. Evaluation and Selection

The evaluation and selection will be based on the criteria set forth below:

A selection committee will review and score each proposal, place the proposals in rank order, and present the results along with their recommendation to the President/CEO for final review and approval.

V. GENERAL CONDITIONS

A. Respondent

All private-for-profit corporations, not-for-profit corporations, local education agencies, governmental units, or public agencies properly organized in accordance with State and Federal law and in business for at least 3 years may submit a proposal. Minority and women-owned and operated businesses are encouraged to submit. A proposal will not be reviewed if (1) the Respondent has been debarred or suspended or otherwise determined to be ineligible to receive federal funds by an action of any governmental agency; (2) the Respondent's previous contract(s) with CareerSource Pinellas or CareerSource Tampa Bay have been terminated for cause; (3) the Respondent has not complied with an official order to repay disallowed costs incurred during its conduct of programs or services; or (4) the Respondent's name appears on the convicted vendor list.

A.  Contract Term and Conditions

The term of this contract shall be from a period commencing on December 30, 2015 and terminating June 30, 2016 with an option to extend for up to three (3) additional years based on performance, business needs and funding availability.

B.  Assignment of Contract

No third party contracts or subcontracts will be allowed, unless specifically approved, in writing by CareerSource Pinellas or CareerSource Tampa Bay.

C. Reserved Rights

The rights reserved by CareerSource Pinellas and CareerSource Tampa Bay, which shall be exercised in its sole and absolute discretion, and shall be at no fault, cost or liability whatsoever, include without limitation, the right to:

·  Supplement, amend or otherwise modify or cancel any provisions set forth in his solicitation at any time.

·  To accept or reject any or all responses, to re-advertise this RFQ, to postpone or cancel this process and to change or modify the project schedule at any time.

·  Disqualify any respondent that submits an incomplete or inadequate response or is not responsive to the requirements of this RFQ.

·  Waive any defect, technicality or irregularity in any response received.

·  Require additional information and/or oral presentation from one or more respondents to supplement or to clarify the proposal submitted.

·  Determine whether the respondent’s written or oral representations are true, accurate and complete or whether the respondent has adequately responded and has the necessary experience, including seeking and evaluating independent information on any respondent.

·  Consider any information submitted that is not requested by CareerSource Pinellas and CareerSource Tampa Bay in a proposal response as supplemental information and not subject to evaluation by the selection committee or President/CEO.
