Instructions
· Full Application is due April 2nd, 2018 by 5:00pm via email to Erica Conte (). Late applications will not be accepted.
· 5-page maximum (not including tables/figures). The cover page, the lay summary, the budget and references do not count towards the 5-page maximum.
· Maximum of two figures or tables can be added.
· Application must be in Calibri, 12 point font. Margins should be 1” on all sides.
· Attach CV(s) (any format) of the PI(s).
· Please include a 150 word lay summary of your research proposal in the indicated section below. This summary should describe your proposal in plain-language that can be understood by individuals who do not have scientific training. Include an explanation of the main benefits of this research to patients, society or the scientific enterprise. This lay summary does not count towards your 5-page maximum.
· The complete application (application form + figure/tables + CVs) should be sent in ONE PDF FILE
