Based on my review of your research proposal, I give permission for you to conduct the study entitled "........" within the ...... organization. As part of this study, I authorize you to invite members of my organization, whose names and contact information I will provide, to participate in the study as interview subjects. Their participation will be voluntary and at their own discretion. We reserve the right to withdraw from the study at any time if our circumstances change.
I understand that the data collected will remain entirely confidential and may not be provided to anyone outside of the research team without permission from the Lindsey Wilson College, School of Professional Counseling, IRB.
