Avalon’s Mission
Avalon, a place of support and understanding for those with memory care needs, has 24-hour skilled nursing care available in a comfortable, safe environment where the focus is on the current abilities of the person.
Avalon recognizes that each person is unique and the support needed may change as the disease progresses. The care approach is resident directed, person-centered which provides our residents with opportunities for joy, comfort, privacy, safety, meaning and purpose all the while being treated with dignity and respect.
Avalon’s Goals
· Avalon will be a calm, accepting environment that is designed to meet the needs of the person, including medical, physical, psychosocial, recreational, emotional and spiritual.
· Avalon will maintain a resident directed, person-centered care focus that supports the resident’s abilities and strengths.
· Avalon’s residents and families will be involved in care planning. We will work with our families and friends to understand the needs and wishes to their loved one. We will incorporate the person’s life story, needs, preferences and abilities into the plan of care.
· Avalon will provide support for our residents and their families throughout the dementia journey with educational and support opportunities.
· Avalon staff will participate in continued education provided by BSJ Care Center. The opportunities will incorporate the resident directed, person-centered mission of Avalon. Further, the objectives of the education include staff developing an understanding of Alzheimer’s disease and related dementia and communication techniques. The benefits are evident when these techniques are observed as part of our ongoing daily interactions with the residents and through the response of our residents.
· Avalon will be committed to providing service and support to those who will best benefit from the support. Avalon recognizes that there will be such time when these support efforts may not benefit a person with dementia. Avalon recognizes the limitations of our program and understands there may be appropriate discharges and/or transfers from the unit due to medical conditions that take precedence or priority over memory care needs.
