Request for New Academic Degree, Major, Track, or Minor
Instructions: A request for a new academic degree, major, track, or minor originates in the academic program’s school. The originator must complete and submit this form to the School’s Curriculum Committee. Once the School’s Curriculum Committee has approved this request and the Dean of the School has signed this form, the Dean then forwards it to the Administrative Assistant for the Office of Academic Affairs to be presented at the next College Curriculum Committee. After the College Curriculum Committee approves the request, the Vice President for Academic Affairs forwards the request to the President’s Cabinet for approval.
Note: The College Curriculum Committee will not review an incomplete request for new academic degree, major, track, or minor form or one without signatures from the appropriate parties.

Enrollment Projections and Monitoring: Provide projected enrollment during the first three years of implementation. Explain the specific methodology used to determine these projections. (NOTE: These projections will be used to monitor enrollment following implementation.)

☐ Are new faculty required to teach courses in the proposed degree/major/track/minor? If it is necessary to hire new faculty to support this request, give the desired qualifications of the person(s) and a timeline for hiring new faculty.
☐ Will current faculty teach the courses in the proposed degree/major/track/minor? If so, provide the name and credentials of the faculty who will teach the courses. Also, explain what impact the new courses in this request will have on faculty workloads. For example, if current faculty are teaching full loads, explain how the new courses associated with this request will be accommodated.
